It is difficult for anyone to advise as you have not provided the ranges .Labs differ in their machines and ranges. With that proviso I will have a brief go at part. It looks like you have Hashimotos or auto immune thyroiditis as GPs prefer to call it. Your TSH suggests your thyroid is struggling .In some parts of the world treatment begins if over 3 but in the UK you will ve lucky to be treated below 10. Based on the antibodies you may be able to get a trial on levothyroxine.
The other thing I note particularly is the very low B12. I think you need to look up Seaside Susies responses to folk with similar results for B12, folate etc. You may also need advice from the pa forum.
Trying to make An appointment is almost impossible I have to wait until next Monday. I'm going to take cytoplan super b vitamins until then. I take fosodopholis for my gut. Just wondered if you think I need t3 supplements ? Do you know anyone who buys them of the Internet and their findings ?

Vit D 93 (50-200)
Almost at the level recommended by the Vit D Council which is 100-150nmol/L.
As you have mentioned bone and muscle aches in your previous post, and now that it is winter, I would be taking some D3 for the next few months eg 2000 or 3000iu daily. As you have Hashi's, an oral spray would be best for better absorption, eg BetterYou. Retest in the March/April with a fingerprick bloodspot test from City Assays vitamindtest.org.uk/
There are important cofactors needed when taking D3
D3 aids absorption of calcium from food and K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems.
D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds.
Magnesium helps D3 to work and comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds
