Why does Endo say ACTH test is not relevant wouldn't a low result indicate secondary Addisons?

I wrote to him to ask for ACTH blood test because they didnt test this when i had my SST but he has refused. See attached letter. I really don’t understand this response particularly as he says even if ACTH is low they don't treat - surely a low result would indicate secondary Addisons?

ACTH and adrenal glands awakening in GCA

Why is synthetic ACTH not used to trigger the adrenal glands into reawakening after a long suppression due to steroid treatment in GCA . At levels of around 4 mg when there is resistance experienced in normal functioning of the adrenals , would not synthetic ACTH stimulate the adrenals ?

How did everyone manage the prednisone free time? What dose were you on at the time. My consult with a endocrinologist was due to low blood sugar readings. Hydrocortisone

I’m taking hydrocortisone but wondering when I should be taking it as I don’t want to completely shut down my ACTH if that’s possible. Is HC addictive and how long could I be on it or until I can come off of it normally?

UPDATE synachten test - morning not possible

This is tricky, if I need to ask for ACTH test. Tiptoeing around making a request and also I am aware I would like to ask for it to be put straight on ice and sent to lab. But I know that is all going to go down like a lead balloon. Patient telling them what to do....

Stress and a Short Synathcen Test

It seems like there may be other alternatives which include the Glucagon test, ACTH and the insulin stress test, or am I barking up wrong trees? Do the NHS readily offer alternatives? Any ideas?

Anyone that could help me understand this?

The next day she called and said my acth levels was a little low and to go see a endrocrinologist. I ended up there and showed her my skin and she said that could be liver disease. She took blood for a fibrosure test and that came back as fatty liver no fibrosis. I was so happy to hear that!

Adrenal insufficiency: a general information post (does not replace medical advice)

In my case my baseline cortisol in all three tests has been above the ‘concerning’ level but below the ‘ok’ level, but then doesn’t rise to the level it should at 30 and 60 minutes despite the ACTH (and my own ACTH levels are fine too), so I have secondary AI.

anything to help me with the tics, but they did do an EKG and a CT scan which both came back normal. i got back home, had another night-long panic attack. i called my doctor and made an appointment to have some blood taken for anything that could have been causing all of that. the lab work was for ACTH

Acth test

I have just had an Short Synathcen test, and included was an Acth test as it's likely that my pituitary is failing. I did check before they took any blood that the Acth test sample would need to be placed on ice and rushed to the lab but the hospital seemed completely unaware of that.

T4 15.8 pmol/L (11-22) T3 7.9 pmol/L 3.1-6.8) TSH <0.01 miu/L (0.49-5.23) TPO 187.6 U/Lml (0-59.0) Thyroid autoantibodies <1.10 UL (0-1.8) Plasma ACTH not done as cortisol in range (wasted trip to hospital) Serum Cortisol 263 (166-507) So I have hasimotos, that I didn't realise I had.

Hello all, I posted my blood tests results a couple of weeks ago: FT3: 4.2 (range 3.5-6.5) (pre Levo level 3.9) FT4: 15 (range 11.5-22.7) (pre Levo level 10.8) TSH: 0.25 (0.55-4.78) (pre Levo level 0.34, but I am TSH deficient because of pituitary issues - also adrenal insufficient because of ACTH

Secondary adrenal insufficiancy??

I find it ironic that the very drug i was given all those years ago which could of caused my long term health issues is the same drug they use to do a 'one off' acth test. Is there anyone else ever been medicated long term with synacthen?
