Firstly , i'm 18 Years old

At first I was a very lazy person inactive etc...

On 15/10/2016 I decided to change my life and go to a nutritionist

I was 96 kg and 34.9% fat

Unfortunately the diet was very poor in calories and vitamins I do not remember exactly how much the calorie was but it was less than 1000 calories

Unfortunately I did not know how to consume and calculating calories and macros and I never put salt on food ever because this Dr told me to not put much salt on food anyway

From 15/10/2016 to 24/2/2017 I lost 26 Kg and my body fat was 19%

But I lost a lot of muscle because of this poor diet and I was not practicing at gym, just walking.

After that I left this doctor and learned the basics of nutrition and calculation of macros and I calculated the needs of the body of protein and carbohydrates and healthy fats and consume it by digital scale

The priority was protein, 40% protein, 40% carbohydrates, 20% fat, and I took 150 micrograms iodine in a day and putting salt on food

Calories: 1600 calories

Food supplements were

Vitamins, fish oil, beta-alanine, keryatine, melatonin (to help sleep), l-arginine, caffeine 400+ per day

In the first four months I became 14% fat and weight 64 kg after that my weight became 65 kg and 13% fat

In the first months of going to the gym I was active and had no symptoms. I was eating 1600 calories every day and the training program was to increase strength

After that I raised my calorie to 2,500 calories for a month and this month I took melatonin to improve the quality of sleep because I slept very little and the stress was too much because of factors beyond my will (i'm not good at dealing with stress)

After a month of calorie raising, I cut calories again to 1600-1700 calories and I consumed a lot of caffeine, probably more than 400 mg a day. My activity increased and my training program updated with new and hard exercise anyway all time i was going to gym 4 days only the session was between 1 and half hour or 2 hours

and i stopped taking Melatonin because metlatonin made me feel dizzy and tired at morning...

I stayed for two months in the third month. I began to feel symptoms of hypo low energy and feel cold or hot very much and after every session my heart beat hard and feel like my heart will jump from my chest

and I was not able to walk after exercise because i was taking my breath heavily and I was suffering from sleep disorders and I was crying after the exercise without reason and also I was suffering from extreme psychological pressure for special reasons and there was pain in my stomach and a slight swelling

In the third month I began to feel the symptoms of weakness and accumulation of fat around the waist and abdomen even with the same amount of calories! I was not losing fat

I know that there is something that is not good I did first analysis on the date of 23/9/2017 at 12 noon

On 26/9/2017 I did blood test for thyroid hormones

Blood Tests : - (T3, Free T3, T4, Free T4, TSH)

Then I went to the endocrine doctor and told me you have bile due to high bilirubin also told me to do a thyroid antibody analysis and re-analyze the other hormones at 9 AM , fasting

He then told me to stop taking any supplements for a month and then do thyroid hormones again after 1 Month

also endocrine doctor give me a treatment for bile.

Note :- i didn't trust this endo anyway he thought that supplements caused Hypothyroidism for me but it wasn't , all my supplements i buy from official stores

- Two days later I did Thyroid Antibodies and other hormones

Date :- 30-9-2017

Blood Tests :- ( LH, Testosterone, Free Testosterone, Antithyroglobulin (Anti TG) Abs, Antimicrosomal AB (Anti TPO) )

Note :- i also stopped the GYM also i didn't did Estradiol (E2) because i thought that if Testosterone is in normal range the Estradiol (E2) will be in normal range also , anyway in another blood test i did again Testosterone and Estradiol again and it was in normal range.

I waited a month and raised calories to 2500 calories

On 29/10/2017 the results of the analysis were found in the level of the testosterone in the level near normal and estrogen also, but was the problem in hormones

TSH , Free T4

Note :- Free T3 is raised a little bit.

I was also suffering from flu symptoms this month and when I did the tests I also had symptoms of flu and colds

And I also did analyzes

Note in this month i gained 10kg in just one month.

Then the doctor told me we would start taking the T4

Dosage:- 50 μg

I began to take the dose on 29/10/2017 , but the surprise!, symptoms were getting worse day by day and in 18/11/2017 I performed blood tests

Note :- my blood pressure is always low , it was normal before being hypo

After that I went to another doctor and told me to take a dose of 75 μg also I did not feel any improvement to be honest some days only I feel very slight improvement and some energy but most of the time or almost all the time I do not feel active and my hair is falling and many symptoms of hypothyroidism

I am now taking a dose of 75 μg and still suffer from the same symptom without improvement or slight improvement and I will perform analyzes on 26-12-2017 to see where am i

i'm now taking Vit. B12 long acting (Hydroxocobalamin) 2000ucg/per week , Folic Acid 2000ucg/per week , Vit B6 40mg/per week, Vitamin D3 5000μg/per day , Iron 100mg per week also i was taking iron pills 50mg per day when i was go to gym , Selenuim 50mg/per day , and some hair vitamins included 150mcg Iodine and my diet is good and eating 2000 calorie per day 40% protein , 40% carbs , 20% healthy fat

My Symptoms for now :-

Dry hand

Pain in the muscles, joints and bones (especially the muscles of the abdomen and chest)

Pain in the joints of the knee and foot

Back pain especially lower back

Weakness and low power

The hair is dry and slightly falling

Depression

Irregular heartbeat

Hard breathing

Insomnia

A little Cold/Hot intolerance

Hungery most of the time or some time (another day) not hungry.

The skin color is pale

Pain in the whole body when waking up from sleep

the two doctors failed to know the reason also me i don't know the reason being hypo :)

So if there is anyone have any advice for me or anyone know the reason of being hypo also please tell me.

Your antibodies are very low, so it's unlikely that you have autoimmune thyroiditis. Therefore, it's impossible to tell why you became hypo. It could have been the iodine you were taking - did you get tested for iodine before supplementing? If not, it's a very bad idea to just take iodine.

In any case, what you need to think about now, is the future. And, at the moment, you are very under-medicated. Your TSH is much too high, and your FT3 is well under-range. You need a higher dose of levo.

Thank you for your reply.

Unfortunately i didn't tested iodine before supplementing, but i was taking 150mcg per day and it's recommended dosage , actually 150mcg it was included with other vitamins i used Opti-Men Multivitamin and Animal Pak Multivitamin , both of those brands have a recommended daily intake usage per day so i didn't overdose of Iodine or Multivitamin,

you're completely right that i should think about now , i'm now on 75 μg Euthyrox the endo tell me to do blood test for TSH , Free T3 , Free T4 on 26-12-2017

actually i didn't trust endo very much but i should follow with doctor so i'm just follow with him for now , i'm also have a T3 25μg but didn't start to take T3 yet so i think if my Free T3 Still low i think to take T3 with T4.

if there is any other advices it will be appreciated.

It's too early to know if you need T3 or not. You need to get your TSH down to about one before you consider that.

Hi - Yup! , you are right , you know i just like being follow with doctor and also read about self-medication for myself to know where is the problem exactly.

i decided also to do Vit B12 , Serum Iron Level , Vit D , Ferritin in

26-12-2017 blood test.

for now as you read i inject myself with Vit B12 and Iron , every week and Vit D3 pill every day and consume RDA of calcium per day also.

Be careful with calcium. That is something you really don't want too much of! Did you have that tested?

When you say 'consume', do you mean in food? Or supplements? Calcium supplements are not a good idea. However, when you are taking vit D3, it increases your absorption of calcium from food. So, you really should be taking some vit K2 - MK7, to make sure that extra calcium goes into the teeth and bones, and doesn't build up in the tissues.

Taking D3 will use up your magnesium, too. So you should also be supplementing that - but six hours away from thyroid hormone! D3 four hours away from thyroid hormone.

i don't take any Calcium Supplement , i just get calcium from food

also i take Omega 3 every day

Oh, goodness! I don't think it's good to be that obsessive about food! You really can over-think things, you know. And I am very anti-calorie-counting.

it's not about calories counting , it's about covering all my body needs from vitamins and macros , if i needs 3000 calories i will eat 3000 calories per day included vitamins and minerals , i don't care too much about calories all i care about vitamins and macros to covering my body needs , that's the point why i'm counting and consume vitamins , macros.

Yes, but you might be missing one very important point, here : you are not what you eat, you are what you absorb. And hypos usually have very poor absorption because they usually have low stomach acid. So, just because you have the perfect diet, doesn't mean you have perfect levels of nutrients, I'm afraid.

actually i do all my best and also i eat 5 small around the day to make sure that i digest my meals well , also all supplement i'm taking its away from my dose i take all supplement after 6 Hours from my dose usually my dose in 9 AM and my supplement is in 2PM or 3 PM and also take digestive enzymes after my meals.

That sounds good. As long as you are respecting certain basic rules for supplementing - e.g. vit D should be taken with fat; iron should be taken two hours away from everything else; magnesium should be taken in the evening because it makes you relax; etc.

Yes i did all of this , i don't use pills of iron , i just inject myself every week with 100mg of Iron.

also i don't take magnesium supplement

Yes, but that's not mean because our stomach have very poor absorption we should eat chocolates, doughnuts every day and don't counting macros and vitamins and just use supplement, we should counting macros and vitamins we taking everyday even calories also to not gain fats very fast...

Sorry, but I feel you are missing a basic point here. As hypothyroid you can have an absolutely ideal diet with plenty of vitamins and minerals and a "macro" balance and yet still have serious deficiencies. Oh and there is no one ideal balance- our bodies all work differently and need differing amounts of fats, carbs, protein, etc so why not ease off a but, be a bit less apparently obsesessive and try to enjoy life?

Monitor the nutrients and supplement as required, continue thyroid blood tests and dose adjustments and come back for advice as required - perhaps without throwing so much at us at once?

i calculate my macros and calories for my only body not just eating a diet from internet!, actually calories counting for body weight and your NEAT , then after counting calories you should counting macros also , absolutely every body need different macros and that depend on the specify calories for every one , but in my situation it's not about calories it's about macros and vitamins that i give to my body as possible

it's a good idea to enjoying life while eating a lot of chocolates etc.. every day without care what my body need, yes i know our stomach have very poor absorption but that's mean that our stomach don't absorption anything! , so i think it will be great idea to do all your best to give your body needs as possible.

actually i eat chocolates , pizza etc , but the main focus on what my body needs and try to give it as possible.

Ah , hmmmm , i don't think that someone write his long story includes every blood test and try as much as possible to know the reason why being hypo and then go to embed malicious links.

Also i don't know how you will be able to enjoy your life while being 20-25% bf and gained 15kg maybe 10kg of just fats and you was 13%-12% bf with a good shape , i think it will be not a good idea also to deviate about your way and enjoy eating junk foods, also if the situation bad it's important to not try to make it worse!

I am not saying you should eat junk food, simply that relaxing your apparently very rigid control over every item you eat may well enable you to enjoy life a bit more in general

Weight and body fat is almost always an issue with hypo and won't usually resolve until treatment is optimum.

Yes , but i should to do that to not make my situation worse , from time to time i eating pizza burgers etc and usually eating chocolates and drinking cocoa.

as you said , i'm just trying to control over every thing because until now i'm under-medicated.

i'v lost 5kg from starting the T4 but those kilograms is just from water retention not body fat.

You should have checked ferritin, vitD, B12 and folate before supplementing. B12 will inevitably be high as you are injecting B12. Probably the same for iron. You would have to stop injecting and supplementing for 4 months to get baseline results. It's still worth testing vitD, calcium and folate in December. Again, calcium should only be supplemented if you have calcium deficiency. The RDA does NOT mean that everyone needs to supplement.

i didn't take any calcium supplement , i just taking my multivitamin and multivitamin include 150mcg Iodine.

So should i stop B12 Injection and Iron Injection and wait 4 Months !?

i will test also for vitD, calcium and folate.

Only if you want a baseline result. You could test and if levels are high stop the injections and retest in 4-6 months.

150mcg isn't the RDA. It is the maximum RDA recommended if supplementation is required. It is not recommended that hypothyroid patients supplement iodine unless they are iodine deficient. Iodine can reduce T4 and T3 and raise TSH. Iodine used to be used to treat hyperthyroid patients. You should be getting the RDA of iodine from the Levothyroxine you are taking and your daily diet.

Your endo is treating you well. Replacing low thyroid is not a quick fix. Chucking high doses at it can cause an adrenal crisis. It's premature to consider adding T3 at this point. Wait and see what FT3 level is when FT4 is in the upper range and TSH 1.0 or lower.

As i said my multivitamins contain 150mcg of Iodine i didn't take Iodine Supplement.

i will wait and see the FT3 Level when FT4 is in the upper range and TSH 1.0 or lower.

also my endo just care about TSH only so that's why i did FT3 and FT4 every time

If there is 150mcg iodine in your multivitamin then you are supplementing 150mcg iodine.

It's entirely up to you whether you supplement iodine but be aware that supplementing iodine has induced autoimmune thyroiditis (Hashimoto's) in patients. I would not take any multivitamin which contains iodine. I wouldn't take a multivitamin anyway. Most do not have sufficient amounts of any individual vitamin or mineral to correct deficiencies and if the multivit contains iron and calcium they may block absorption of other vits/minerals.

If you type "Iodine" into the Search Thyroid UK box you will find thousands of posts about iodine. It is a very contentitious topic.

Yes you are completely right Iodine it's a very contentious topic.

The ranges of multivitamin i used now include very low ranges of basic vitamins so i don't worry about the multivitamin very much.

Sys3m3rror,

50mcg has raised FT4 but FT3 remains below range. It is low FT3 which causes hypothyroid symptoms. Your dose needs increasing in 25mcg increments at 4-6 week intervals until FT4 is half to 2/3 through range and FT3 at least halfway through range. TSH will usually be 0.48 - 1.0 at that point. Thyroid antibodies are negative for autoimmune thyroiditis (Hashimoto's). You are unlikely to find improvement in symptoms until FT3 is within range. It's a slow business restoring thyroid levels.

It takes 7-10 days for Levothyroxine in a dose increase to be absorbed before it starts working and it will take up to six weeks to feel the full impact of the dose. Symptoms may lag behind good biochemistry by several months.

For maximum absorption Levothyroxine should be taken with water 1 hour before, or 2 hours after, food and drink, 2 hours away from other medication and supplements, and 4 hours away from calcium, iron, vitamin D supplements, magnesium and oestrogen.

Symptoms of hypothyroidism are in the link below. Some of your symptoms are likely to be due to anaemia. High cholesterol is commone in hypothyroid patients and should improve as your thyroid levels improve. It is a good idea to reduce your exercise until FT3 is within range as exercise depletes T3 and that is why recovery from exercise can take several days. Gentle exercise such as walking and swimming are recommended in the meantime.

Did you check ferritin, vitam D, B12 and folate levels before supplementing iron, D3, B12 and folate?

Liver function may also be impacted when thyroid levels are low. They may also improve when thyroid levels improve.

Tissue transglutaminase is negative so you don't have coeliac disease.

i just up my dose from 50mcg to 75mcg from two weeks , i will do blood test of TSH , FT4 , FT3 again on 26-12-2017 and will see what 25mcg did to me.

Unfortunately i didn't check ferritin, vitD3, B12 and folate levels before supplementing and i was taking 10000μg vitD3 per day before being hypo because i was not expose to sun very much but i reduced it now to be 5000μg

i will let you know the results once i did all blood test again.
