Blood work results.

Good morning. I finally got all the results the endo asked me to bring with me on Thursday.

My cholesterol is high but I won't bother you reading it. I'll put the reference values in brackets.

My sodium is 131 (135-146)

Vit D-OH total: 29 (30-100)

Vit D-OH D3 : 29

Vit D. 25 OH, D2 : <4

Testosterone : 40 (2-45)

TSH: 0.51 (0.4-4.5)

T4 Free: 1.4 (0.8-1.8)

T3 Free: 2.3 ((2.3 -4.2)

Estradiol: 179 (Perimenopause. it does not provide the levels for this season)

Prolactin : 8.3 (non pregnant 3.0 - 30.0 )

DHEA Sulfate: 68 (8.0 - 188)

VITAMIN B12: 478 (200 - 1100)

Folate Serum: 12.7 (Normal >5.4)

I forgot to ask for Ferritin. The doctor did not order the vits but I did.

Any recommendation will be very well received,

TSH: 0.51 (0.4-4.5)

T4 Free: 1.4 (0.8-1.8)

T3 Free: 2.3 ((2.3 -4.2)

if you feel OK .... these results show 'no need to change dose', there is no problem with TSH at that level.

if you don't ... the low T3 is very probably the reason.

(And Vit D looks deficient , but i don't know much about Vit D results )

That is my fear. That he ( the doctor) will want to change the dose. Thank you.

Well as long as all results are 'in range' , he has no grounds whatsoever.

Now if TSH was 0.39 [0.4-4.5] they might well start banging on about "Risks of low TSH" , but if it's in range .. it's in range ..... and the range is made up of 97.5% of the healthy population.. so (according to them) it must be alright .

Of course some of them might still try... but you have every reason to decline because your TSH is inside the lab reference range

Thank you. I hope somebody else could help me with the free t 3 conversion. I am seeing the doctor on thursday and I am really horrified. And it is not a joke.

A few thoughts:

So, your FT4 is 60% through range, but your FT3 0%.

First, your conversion seems poor. With such a low FT3 you must feel awful. I know I did when mine was bottom of the range.

Second, your FT4 could be higher. Mine needs to be around 75% of the reference range to feel functional.

I would say you either need more T4 or some T3, or both.

Meantime, while I don’t understand the nuances of vitamin levels, I’d suggest all of yours could be higher, especially vitamin D. All of these support the body’s use of thyroid hormones.

When you say you’re horrified about seeing the doctor tomorrow, what do you mean?

Sorry. Just read your previous post. You must not let your doctor prescribe based on TSH here. He’s going to keep you quite ill if so.

Thank you. He blames high cholesterol on me or my genes. Same as my weight. And, he will blame everything in perimenopause. I like him, he is the best I have found so far. But, I do not get feedback from him. For instance, if I ask him about considering selenium or others his answer is " Don't you dare". So,....... I go because I need the prescription. He is very expensive but a gentleman and I appreciate that. Thanks again.

I hear you. All my symptoms were for years blamed on menopause, high cholesterol or depression. I changed doctors in the end. It’s very difficult to be heard and be well treated with thyroid issues. But don’t give up trying 🌸

YourT3 is on the bottom rung of the range - and it is T3 that gives you your wellness not T4:

Your body needs to covert the T4 into T3 and your results show you struggling :

I read ferritin needs to be over 70 for any thyroid hormone replacement to work well, and I now maintain my ferritin at around 100 : folate at around 20 : serum B12 500 ++ and vitamin D at around 100 .

What have you actually been diagnosed with and is there any other thyroid hormone replacement option readily available in your country as I don't think you're in the UK are you ?

I wish I was in the UK. So pretty landscapes over there. I am in USA. I am not saying I do not like it here but, I do love the polite way of being you have.

First step is to get all four vitamins optimal

What vitamin supplements are you currently taking

Vitamin D at least around 40ng/ml (100nmol in U.K. units)

Likely to benefit from supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B or Jarrow B Right are recommended options that contains folate, but both are large capsules. (You can tip powder out if can’t swallow capsule)

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

Obviously essential to test iron and ferritin levels

Do you have autoimmune thyroid disease also called Hashimoto’s diagnosed by high thyroid antibodies

Do you take levothyroxine waking or bedtime. Always on empty stomach and then nothing apart from water for at least an hour after, no other medications or supplements within 2 hours minimum

Was test done after minimum 8 weeks on constant unchanging dose levothyroxine

Was test done as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test

When adequately treated we should have Ft3 at least 60% through range

Thank you so much, Slow Dragon. I would say yes to all of your questions. I stopped taking Vitamin D and B complex like two weeks before the test. My hemoglobin and hematocrit are in the lowest normal. ( I think they have something to do with Iron). II take the pill as soon as I wake up and I really wait for 1/2 an hour before coffee. Thank you so much. I ordered the tests online and I was told Iron wa going to be measured.

My folate accordig to the lab is high. (12.7 ng/ml) According to the place that wrote the report, it is normal.

Try taking levothyroxine at bedtime (minimum 2-3 hours after a meal)

Can be more effective

Or try splitting dose…half dose waking and half at bedtime

Thanks again. At night, I normally take something for digestion. That is why I have never tried that schedule. Thanks. Tomorrow is the famous appointment. It has me neurotic but people like you and the community as a whole help a lot.

Well I would have thought the USA was more open to prescribing other thyroid hormone replacement options as in the UK we are routinely only offered T4 - Levothyroxine.

You might fair better out of " mainstream " as I understand chiropractors, and integrated, more holistic doctors seem more " enlightened " from what I 've read.

Think again, Pennyannie. There are so many "merchants" out there. I have also read what you have and I have visited two that were more interested in selling their products than my wellbeing as a patient. And chiropractors cannot prescribe medication.... Thank you, Xiflo

Thank you everybody for your input. I went ready to fight and the doctor was open for changes. LOLHe added some T3 but, at the same time he advised to be careful with it due to my age changes. (perimenopause) I was so stressed that I have lost sleep over it. Thank you again, God bless everybody.
