Help with New blood Results. Now what do I do Since I just started Armour and numbers getting worse

New blood results from 2-12-19 collection.

TSH 9.77 (.4-4.5)

Free T3- 2.8 (2.3-4.2)

Free T4- .9 (.8-1.8)

RT3- 8 (8-25)

My blood results before that was taken on 12-2-2018

TSh 6.90 (.4-4.5)

Freet T4 1.1 (.8-1.8)

Free T3 2.8 (2.3-4.2)

RT3- 10 (8-25)

I switched to Armour thyroid on 1-8-2019 90MG. Before that I was on Synthroid 112 Mcg and Lirothyronine 10MCG a day. I was on that for at least 6 months. I started at a functional Hormone Dr. beginning of Dec 2018 that is why I has the blood tests then. So we thought we were doing the right thing to switch to Armour 90mg to get my numbers to where they need to be. But they are getting worse. What to do? They have not gotten to me yet about these latest results.

I currently take the 90mg armour early in am.

I take LDN 1.5MG at night.

I take through out day, B12, Vit D3 with K2, Vit C 1000mg, Magnesium G at night, Omega 3, Tumeric, and CBD oil. Eat pretty healthy and not 100% Gluten free yet but Dairy for a few months.

Current symptoms - Lower and middle back pain since August, Fatigue, Rib pain, vision gets blurry when trying to read things.

I did have high calcium at 10.6 (8.6-10.3) on a blood test early January. (Regular calcium blood test)

I did do a follow up Calcium Ionized a week later to double check calcium and it was 5.3 (4.8-5.6). (More in depth test I guess compared to regular calcium test).

I dont think I have ever gained weight at all with hashimotos to this point. I can eat a lot but be hungry pretty quickly. I am actually down a pound or two over past two weeks.

My Vit d was at bottom of range in December even though I was taking 5000IU Vit D 3 with K2 every day for months but since stopped doing that after my last Vit D test because my calcium was over range in the standard calcium test.

Am i under medicated? I thought 90mg would easily help compared to the 112 Synthroid and the 10mcg of Lirothyronine.

Whats causing this

90 mg NDT only contains 57 mcg T4, so yes, you've given yourself quite a reduction in dose. However, the FT4 has only gone down a little, and the FT3 not at all - despite the slight increase of 3.5mcg T3 - which is pretty much nothing at all. It's the TSH that has risen. But, as I see it, that could have happened anyway because you were very under-medicated on the T4+T3. Changing to NDT was a chance to give yourself an increase in dose, but starting on at least 2 grains - which would only have been 76 mcg T4 but 18 mcg T3. From there, increase by 1/4 grain every two weeks until you feel well.

Your dose of thyroid replacement is much lower that your previous levo and T3 you need to gradually increase the NDT. Your T4 will drop anyway, it does with NDT that is expected and normal but you need to be aiming to get your T3 top of the range. TSH has risen I expect due to your drop in dose.

When you get your dose correct the TSH will be suppressed and the FT3 high in the range but never go over and FT4 may lower. Increase slowly though or you may miss your sweet spot.

This is the response from my Hormone/Functional Doc. on private email with concerns to my latest results as shown above.

--->" I don't want to increase his armour thyroid just yet. His adrenal fatigue might be causing the issue with the increased TSH. I would like to try starting him on Adrenal Manager take 1 in the morning and 1 at lunch, if no improvement within 5 days he can take 2 caps in the morning and 1 at lunch. If he has no response to the adrenal manager within a week I will prescribe cortef for him to take in the morning and at lunch."

-What are you thoughts? My 24 hr Saliva test two months ago was as follows.

cortisol am- 2.28 Range (1.8-3.8)

Noon .31 (.3-1.4)

Evening .14 (.1-1.1)

NIght .19 (.1-.8)

So my cortisol is at the bottom of range for all times.

I'm not sure that low cortisol can cause the TSH to rise independantly of the Frees. The TSH has risen because you're under-medicated.

I don't know Adrenal Manager, but looking at its ingredients, it's a bit of a mish-mash. Not sure how much use it would be to you, but worth a try. But, I'm not sure your cortisol levels are low enough to warrant cortef. That's pretty strong stuff! Your cortisol is about mid-range in the morning, which isn't ideal, but it's not that low. And, for the rest of the day, cortisol is supposed to be low in range. You need it most early morning. So, let's hope the Adrenal Manager does its stuff! :)
