Help with blood test results - thyroid / sjogren's syndrome

Looking for some advice on my latest blood test results - I posted a few months ago as I have suspected sjogren's syndrome, but I wanted to investigate the possibility of thyroid issues due to family history. My Dad has grave's disease and rheumatoid arthritis and his mum also had RA. My symptoms are very typical of sjorgen's - severe dry eyes, mouth, nose & everywhere else. Terrible fatigue & exhaustion and brain fog, joint and muscle pain, muscle twitching, painful / swollen lymph nodes, a flu type feeling when i'm most tired and most recently sun sensitivity rashes. I am 30 and have been suffering for 5 years with my eyes but have been feeling very poorly since January this year. I also suffer from a terrible racing mind / anxiety, irritability, insomnia, slow reactions, headaches & noise sensitivity, occasional hand shaking & lump in throat which is why I wanted to see if I could possibly have Grave's disease either instead or as well as sjogren's... my Dad has always said I am showing the same signs as he did when he first got ill with Grave's (apart from the dryness)

My results are as follows:

TSH 0.27 - 4.20 mIU/L 1.950

Free T4 12 - 22 pmol/L 15.73

Free T3 3.1 - 6.8 pmol/L 4.4

Total T3 1.3 - 3.1 nmol/L 1.7

Total T4 59 - 154 nmol/L 90

TSH Receptor antibody < 2.9 U/L 53

rT3 (by LS/MS/MS) 10 - 24 ng/dL 12

Thyroid Peroxidase antibody 0 - 24 IU/mL 11

Ferritin 13 - 150 µg/L 19.26

Total Vitamin D 75 - 200 nmol/L 56

Serum Folate 3.89 - 26.80 ng/ml 16.08

Vitamin B12 197 - 771 pg/ml 811.8

Zinc 9.6 - 20.5 µmol/L 13.70

Selenium 0.75 - 1.46 µmol/L 0.92

Magnesium 0.7 - 1.0 mmol/L 0.80

These were commented on by a GP via email who said I had a positive result for TSH receptor antibodies which may lead to Grave's disease in the future and also I am slightly deficient in vitamin D and Ferritin, however my B12 is over range quite a bit. I had stopped all supplements for 1 week prior to testing and these were taken in the morning after fasting.

Due to all my TSH, T3 & T4 levels being pretty much in the middle of the ranges, does this mean I don't have Grave's disease yet or do positive antibodies mean I do?

I am due to see a rheumatologist on Monday (a lady very experienced in sjogren's) so I will hopefully get an answer soon, but is it even worth seeing my GP about my thyroid results if they are all still within range? They have always been very dismissive and unhelpful previously. I had to push for thyroid tests last time and was told my symptoms are very vague and suggested I just had depression!

I would wait until you have feedback from the lady you are waiting to see then start another thread. It's so close so better to add her input as well.

If you’re seeing a very experienced Sjögrens lady...she will work it out for you. Good luck!

I sent that before I’d finished! Sorry. All the symptoms you mention sound like Sjögrens...but she will be able to check that by looking at ANA and specific antibodies in your blood. I am not a doctor, but have Sjögrens and the lady I see is very good at explaining everything and giving me treats options. Let us know how you get on. 😊

Thank you. My last ANA was negative but I had a low positive a few years ago, never been tested for anti ro or la but hopefully she will test tomorrow. Will let you know how I get on :)

I have Hashimotos and have had very similar symptoms to yours a few years ago. Dry eyes, very red and extremely sensitive to light. Fatigue and brain fog, joint and muscle pain, a flu type feeling at the worse times. Swollen face and a terribly itchy rash, mostly n places that were exposed to sunlight but that would spear elsewhere so I am still not sure whether it was due to light or possibly to a gluten allergy (I have found since that I was sensitive).

It looks many months for these symptoms to settle down. I wonder now whether I went through a hyperthyroid phase. Doctors were useless. They dismissed the possible connection with my thyroid and did not investigate me for this.
