Low, and I can't take INUVI's reference range seriously considering if Medichecks had sent this test to County Pathology the range would be 13-150. I see from previous posts you're having trouble raising this as a vegetarian.

Quite a nice level, folate is recommended to be at least half way through range which yours just about is. It won't hurt to continue with the B Complex.
Vitamin B 12 -active 92 pmol/L (37.5-188)
I stopped taking vitamin B12 when my results reached optimum levels I have noticed it has dropped. Is there a maintenance dose I should take?
Your level appears to have dropped from 138 to 92 in the space of 4 months since stopping B12 but due to the change in range we should look at percentage.
Currently 36.21%, previously 89.33% so that's quite dramatic. If this was me I'd start the B12 again and perhaps take alternate days and retest in 3 months. Of course you will need the B Complex as well as it should be taken when taking B12.

Vitamin D 3 +K2 100 ug daily and magnesium citrate.
The Vit D Council, Vit D Society and Grassroots Health all recommend a level of 100-150nmol/L with a recent blog post on Grassroots Health recommending at least 125nmol/L.
What is the dose of D3 that you take?

Shows poor conversion, FT4 is 52% through range with FT3 just 13.51%. Increasing ferritin should help, some experts say the optimal ferritin for thyroid function is 90-110ug/L.

TPO antibodies confirm Hashimoto's which you're aware of.
What do the changes in ranged mean?
It means they may have recalibrated the testing equipment. Nothing to worry about, it's where in range your result lies not the actual range itself. If you're comparing a result with a previous one and the ranges have changed then you work out the percentage through range, here is a calculator:
No, no need to increase. You might even need less if you make Vit D naturally from the sun. Do you know if you do? But it might be worth continuing with that dose for now, retest end of October to check your level to make sure you don't need to adjust dose for the winter months.
Ok thanks . I don't get much sun as I am on metherexate for rheumatoid arthritis advice is to stay out of sun on this treatment I am also a freckly red head so have never been in the sun much
you will see they recommend 2,500iu D3 to reach 50ng/L (125nmol/L_ and 4,600iu to reach 60ng/L (150nmol/L) so continuing with 4,000iu seems about right for now.
Thanks again for this. I have to say I am feeling so much better following the advice from everyone here and have seen improvements slowly but surely. I realise it is ongoing and a work in progress and I need to keep on top of this self care.
