I hope someone can help me. I have Hashi and my main issue these days is malabsorbtion of protein and fat. I am low on some nutrients that I now supplement with, such is B vitamins in methylated form, Iron, selenium, zinc, Magnesium, vit D and C.

However, I cannot seem to resolve the malabsorbtion when it comes to fat and protein and as a result, despite eating a lot, I am getting anorexic looking and my hair is falling out. I have such a low self-esteem because of this and worry constantly.

I am taking HCL & Pepsin 1 capsule with each meal ( biocare) and Gallplus ( Nutri Advanced) and regularly consume kefir and saukrat. Eat very clean. However, I continue to have these problems.

I am working towards optimising my T3, taking Thyroid-S.

My latest results below:

TOTAL THYROXINE(T4) 69 nmol/L 59 - 154

THYROID STIMULATING HORMONE * 0.04 mIU/L 0.27 - 4.2

FREE THYROXINE 14.3 pmol/l 12.0 - 22.0

FREE T3 4.3 pmol/L 3.1 - 6.8

THYROID ANTIBODIES .

Thyroglobulin Antibody * 227.0 IU/mL 0-115

Method used for Anti-Tg: Roche Modular

Thyroid Peroxidase Antibodies * 145.0 IU/mL 0 - 30

Feel tired and wired, have trouble sleeping, getting weird morning sweats.

I was seen by NHS endocrinologist, did a bunch of tests, endoscopy and colonoscopy that showed lower oesophagus hiatus hernia and hemorrhoids. Also slightly elevated AST at 50 (0-35). No treatment was given and after the checks all my symptoms were simply ignored.

I am willing to go private now. I live in Bishop's Stortford but happy to travel a reasonable distance.

Please help!

Was the thyroid test done as advised here, testing as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test

Day before test split T3 into 2 or 3 smaller doses spread through the day, taking last 1/2 or 1/3rd approx 8-12 hours before test

Remember to stop taking any supplements that contain biotin a week before all blood tests (eg in vitamin B complex) as biotin can falsely affect test results

When were vitamin D, folate, ferritin and B12 last tested

Have you had coeliac blood test/ endoscopy

Are you on strictly gluten free or dairy free diet

Hi, yes followed all the instructions precisely! I am gluten free and diary free. I had endoscopy, no issues apart from small sliding hernia. :-)

How much levothyroxine are you currently taking Which brand of levothyroxine

How much T3 and do you typically split dose through the day or take single dose

FT4: 14.3 pmol/l (Range 12 - 22) 23.00%

FT3: 4.3 pmol/l (Range 3.1 - 6.8) 32.43%

Helpful calculator for working out percentage through range

thyroid.dopiaza.org

Many people on levothyroxine and small dose T3 find they need both Ft4 and Ft3 at least 50% through range

Your results suggest possibly under medicated

Do you make sure iron supplements minimum 4 hours away from levothyroxine and T3 and other supplements

Magnesium 4 hours away from levothyroxine and T3

I am Thyroid-s only. 1 in the morning and 1 at 5 pm. Thank you for the link! I do leave at least 4 hours between Iron, Magnesium and zinc. Whats the target T3? Is it upper quarter? Thank you.

Very individual

On NDT Ft4 will always be low

But your Ft3 is probably too low
