Good afternoon

Could I have someones Opinion on my test results please all were taken as a fasting with no levothyroxine taken until after my blood was drawn at 8.05am.

I did ask for T3 but not there

Haemoglobin concentration: 141g/L [120.0 - 150.0]

Platelet count - observation: 335 10*9/L [150.0 - 410.0]

Red blood cell count: 4.98 10*12/L [3.8 - 4.8]; Above high reference limit

Haematocrit: 0.42 L/L [0.36 - 0.46]

Mean cell volume: 84 fL [83.0 - 101.0]

Mean cell haemoglobin level: 28.3 pg [27.0 - 32.0]

Mean cell haemoglobin concentration: 335 g/L [315.0 - 345.0]

Red blood cell distribution width: 14.0 % [11.6 - 14.0]

Neutrophil count: 5.3 10*9/L [2.0 - 7.0]

Lymphocyte count: 2.4 10*9/L [1.0 - 3.0]

Monocyte count - observation 0.4 10*9/L [0.2 - 1.0]

Eosinophil count - observation 0.2 10*9/L [0.0 - 0.5]

Basophil count: 0.0 10*9/L [0.0 - 0.1]

Nucleated red blood cell count 0.00 10*9/L

Pathology Investigations B12:  Serum vitamin B12 level 293 ng/L [197.0 - 771.0]

Serum ferritin level: 181 ug/L [13.0 - 150.0]; High ferritin can indicate true iron overload,inflammation, dyserythropoiesis or liver disease(including increased alcohol intake). Tissueoverload can occur if ferritin exceeds 1000 ug/L.; Above high reference limit

Thyroid function test:  Serum TSH level 4.3 mIU/L [0.3 - 5.5]

Serum free T4: level 14.5 pmol/L [10.0 - 22.0]

VITAMIN D:  Serum total 25-hydroxy vitamin D level 67.2 nmol/L [> 50.0]; 50 nmol/L: sufficientVitamin D falls in acute illness and post surgery.

SERUM FOLATE:  Serum folate level > 20.0 ug/L [2.1 - 26.8]

Personally I find it too difficult to read blocks of text where the results all run into each other like that, but if they're one test on one line I'll be happy to respond.

Looking at your profile

currently on 125mcg of levothyroxine 3 days a week and 100mcg the other days

Are you still on this dose?

Which brand of levothyroxine

Results show you are not on high enough dose

Perhaps increase to 125mcg daily

Retest in 6-8 weeks

Are you currently taking any vitamin supplements?

Vitamin D too low

Aiming for at least around 80nmol

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and add a separate vitamin B Complex after a week 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) 

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) 

Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

Thank you I will read through all the information you have supplied.

i'm still on the same dose of levothyroxine I only take wockhardt (or insist) the other brands make me feel unwell.

I'm not on any vitamins at the moment ,I've tried the vitamin B12 from cytoplan in the past and it sadly upset my stomach I also suffer with Crohn's disease {I'm in remission} I have a lot of symptoms and I do think its down to low vitamin b12 I just need to work out how to solve it.

GP should test for Pernicious Anaemia before starting you on either B12 injections or daily B12 supplements

If GP disinterested…..start taking daily B12 supplements yourself

Retest levels in 6-8 weeks ……and after increasing levothyroxine too obviously

My dr won't entertain any of it I'm fine as I am apparently (which I am far from fine at the moment)I am looking into changing surgery as it's having a real effect on me this constant fight I have started taking the Bioactive B12 drops and will retest soon thank you for the advice

Conclusion

Vitamin D insufficiency in IBD remains common. Consensus expert opinion has suggested 25(OH)D levels of 75–100 nmol/l may provide optimal benefits for musculoskeletal and cancer outcomes( Reference Souberbielle, Body and Lappe 20 ) and levels of 100–175 nmol/l for optimal immune effects( Reference Cannell and Hollis 113 ). The data reviewed here show evidence of positive associations with levels ≥75 nmol/l in Crohn's disease, and further possible associations with levels ≥100 nmol/l but such associations need validation with well-designed randomised controlled trials.
