Test results advice

Hello,

I wondered if you might be able to advise me on my recent test results from Thriva?

Blood taken at 11.30 am fasting. I had taken Levo at 10 pm previous night.

I'm taking Solgar sub lingua B12 which includes folate - do I need to stop this and change to just B complex to bring down B12? Is there any danger to high B12?

I currently take:

125 mg Levo each day

Vit D with K2 Better You spray 6000/day

B12 w B complex solgar sub lingua drops

About to start supplementing with Selenium, Folate, Magnesium, Iron, Zinc and return to 100% gluten free diet and re-test in 3 months.

Any other recommendations very welcome from this super knowledgeable group.

Thank you!

TSH 0.92 mIU/L (0.27-4.2)

FT4 18.1 pmol/L (12-22)

FT3 3.4 pmol/L (3.1-6.8)

TPOAb 147.2 kIU/L

T4 115 nmol/L (66-181)

TgAB 26.4 kul/L (0-115)

CRP 1.53 mg/L (0-3)

Ferritin 45 ug/L (15-150)

Vitamin D 109 nmol /L (50-200)

Active B12 150 pmol/L (37.5-150)

Folate 11.2 nmol/L (8.83-45)

If you are supplementing then a raised B12 is what should be expected. It doesn't mean it is dangerous.

But as your write-up says, if you are not supplementing B12 then high B12 could indicate a problem which needs to be looked into.

I am supplementing. I wondered if I needed to reduce my supplementation?

Active B12 150 pmol/L (37.5-150)

There's no need to continue with a separate B12. Once your level has reached 100 for Active B12 then a B Complex, which often contains 400mcg B12, should be enough.

Suggested brands:

Thorne Basic B (although often out of stock and can be expensive)

Yipmai Liposomal B Complex softgel from Amazon - amazon.co.uk/Yipmai-Liposom...

Vitablossom Liposomal B Complex softgel (exactly the same as the Yipmai just branded differently) - hempoutlet.co.uk/vitablosso...

High B12 from supplements isn't a problem, it's just due to taking more than you need.

About to start supplementing with Selenium, Folate, Magnesium, Iron, Zinc

How much selenium are you planning to take? Have you tested?

A separate folate supplement may not be needed, I'd see how you go with the B Complex, all of the above brands contain 400mcg methyfolate and that might be enough to raise your level. Aim for mid-range or higher.

Have you done a full iron panel (serum iron, transferrin saturation percentage, total iron binding capacity, ferritin) to know that you need it. Don't take iron tablets purely based on low ferritin. If your serum iron and saturation percentage are already at good levels then taking iron tablets will increase them and may lead to iron overload. If serum iron and saturation are good then you can help raise your level by eating liver regularly, maximum 200g per week due to it's high Vit A content, also liver pate, black pudding, and including lots of iron rich foods in your diet

Have you tested zinc and copper to know that you need zinc? We hypos tend to have one high and the other low and we should find out what we need before supplementing.

Blood taken at 11.30 am fasting. I had taken Levo at 10 pm previous night.

TSH 0.92 mIU/L (0.27-4.2)

FT4 18.1 pmol/L (12-22)

FT3 3.4 pmol/L (3.1-6.8)

The timing of your test has given you a lower TSH than if you'd done the test at 9am.

We advise last dose of Levo should be 24 hours before test (adjust time day before if necessary). By taking your Levo at 10pm the night before your 11.30am test you have a higher than normal level of FT4 than that which would show as your normal circulating amount if you'd left 24 hours.

FT3 is very low suggesting conversion is poor. Optimising ferritin and folate levels might help. I'd retest about 3 months after starting supplements to see if there's any improvement. If not then the addition of some T3 might be of benefit.

Don't start all supplements at once. Start with one, give it a week or two and if no adverse reaction then add the next one. Again, wait a week or two and if no adverse reaction add the next one. Continue like this. If you do have any adverse reaction then you will know what caused it.

Thank you, have taken on board all your advice and ordered suggested B Complex & blood tests for Selenium, Copper, Iron panel, but cant find zinc test - do you know where this might be available?

Thanks again!

I had zinc tested years ago, it was with Blue Horizon but an added freebie when I had a very comprehensive test. I also, more recently did the mineral deficiency test with Cerascreen and this included zinc

As far as magnesium is concerned testing magnesium is unreliable if it's a serum magnesium test. About 99% of magnesium is stored in bone, muscles and soft tissues, leaving about 1% in the blood. So testing what's in the blood isn't giving an accurate picture of our magnesium status.

A red cell magnesium test is the better indicator of magnesium status, not the standard serum magnesium test. The red cell test is expensive and requires phlebotomy which is why it's not included in these bundles.

A gluten free diet helps some people but there is no guarantee. I've not tried it myself.

Thank you so much, this is so helpful. I will do a full iron panel and also test for zinc / copper and selenium - do you know where I can test for these? And can you test for magnesium levels?

I will change to a B Complex w folate rather than the B12.

I am aware of poor conversion and hoped these supplements might help but will do so slowly as you suggest.

Can gluten free diet help with conversion or just reduce antibodies?
