Recent blood tests- need advice

Case is now in its 41st year, would very much appreciate your advice and recommendations as brain fog is worse now than ever- extremely depressed coupled with constant anger.

Recent blood tests done by NHS lab in Royal Hospital Liverpool got total backing from GP and included many tests not relevant to thyroid and adrenal gland but which might cause Chronic Fatigue .Unfortunately his knowledge is limited.

Blood test taken Mon 26th Feb at 9.45am. I did not take T3 or T4 for 24 hours before- B complex tablets contain Biotin.

I have worked hard on supplements B12, Vit D , Folate, Ferritin, B Complex and Selenium but brain fog is worse now than ever - - whatever is wrong with me I feel is not psychological although NHS says it is and is definitely not the thyroid. Results of tests done Feb 26th 2018

TSH <0.05 0.30 to 5.00

T4 7.4 Range 11.00 to 22.00 Note from lab ? Patient on T3 treatment

T3 4.7 Range 3.10 to 6.80

B12 1037 Range 181 to 910

Vit D 199 Range 75.00 to 150.00- received call from doctors whatever dose you are taking reduce - started on 10000iu reduced to 5000iu considering dropping to 1000iu per day

Folate >24 range >5.40

Ferritin 79 Range 22 to 322

Adrenal gland

Cortisol 307 Range 145 to 619

DHEA Lab refused to test stating of limited clinical utility in males. Book by Dr Barry Peatfield mentions DHEA many times adrenal gland very important particularly DHEA

Since blood test in December 2017 by Medichecks, I continued with 100 mcg of Thyroxine whilst supplementing with B12 Vit D Folate Ferritin and Selenium - no improvement obtained Unipharma T3 from Greece started on 6.25 increased to 13mcg after 2 weeks increased again on 14th Feb to 25mcg and reduced Levo thyroxine to 50mcg.

Had Blue Horizons Genetic test special offer 5 weeks ago results out next week.

Will cease Unipharma T3 in 1 week's time- due to receive NDT from Thailand soon will try that .

Need to keep working on Iron did try liver but reluctant to carry on research shows cholesterol in it sky high also trans fats . Will find better source of Iron.

Planning on having another blood test with Medichecks end of March- I will include full tests on Adrenal Gland -they test with saliva rather than NHS blood sample .Will also include as much as possible relevant to Thyroid and Adrenal gland.

Need advice should I try Unipharma for longer- drop it altogether -stop taking Levo -

try NDT on its own .

Having left 24 hours between last T3 dose and blood draw you can extrapolate FT3 by +20% to estimate 5.64 as your normal circulating FT4. This is in the upper third of range which is adequate for most people but there is scope to increase dose by 6.25mcg to see if symptoms improve.

FT4 is low because you are taking proportionally more T3 than Levothyroxine.

I would drop vitD to 1,000iu as 199 is high and continuing 5,000iu could cause toxicity.

Ferritin 79 is adequate but halfway through range is optimal. It's better to eat liver once a week and other iron rich foods than to supplement iron.

B12 and folate are good.

It's advisable to stop supplements containing biotin 4-5 days prior to blood testing because if the lab uses biotin in assays results can be skewed.

I have restarted taking Vit D at 1000iu coupled with Vit k2

Will try Liver for sometime but cholesterol amount does worry me. Researched iron this morning absorption rate is 35% from liver 2% from supplements.

Been taking T 3 Unipharma for 4 weeks no improvement increased brain fog but not sure what is causing it T3 or too many supplements. I am taking 25 mcg of T3 are you suggesting I increase it by another 6.25mcg. How long does it take to show improvement or is that impossible to say.

Which form of T3 do you take?

2 weeks 12.5mcg and 2 weeks 25mcg isn't long to expect much improvement. I was suggesting you try an extra 6.25mcg but if 25mcg made brain fog worse there doesn't seem much point.

I had improvement in brain fog and mood within a week of adding T3 to Levothyroxine but it took months to improve physical symptoms. I'm prescribed T3 and have taken Mercury Pharma and Morningside T3. I've also self medicated Tiromel.
