Stage 4 A Prostate cancer

was 11. Has a history of prostrate cancer. Was wondering if we should get another opinion regarding getting surgery first. The dr wants to treat it aggressively. Any advice on what to expect, next steps appreciated. More Information: Hi father died of prostrate cancer as did his uncle.

I had my prostate removed 2 years ago at MSK and have recovered well with a still undetectable

My husband was DX April 2021 MPc, Stage IV,

335, BRACA2. Lupron, Erleada, Provenge, Lynparza (still on), Zytiga/Pred (still on).
PSA
rose from 2.64 Feb 2023 to 44 Sept 2023. Biggest jumps on the last three months, 10 to 44.

Furthermore, prior to starting ivermectin,

would shoot to to 6.7-7.7 when I would get off adt. I’m not saying this is for everyone but it’s thus far working for me after 9 months and something possible to look into especially when

is in its infant stages and pca oligo metastatic.

Are Lymph nodes a filter?
When I took the ADT, my
PSA
went from 1000, down to 6, and all evidence of cancer disappeared from my lymph, pretty much immediately.

Failed Pluvicto
My
PSA
is now rising rapidly and scans show many new mets and growth in old ones, from skull to feet and everywhere in between. I'm in my 80's and wondering how much time and when this will get painful. I don't want any more aggressive treatments that might get me a few more months.

RP Dr V Patel march 2020 (T3b, negative margins and negative lymph nodes) and

undetectable until Sept 2023. Started at 0.021 and now Feb 22nd 2024 at 0.052. Seeing Dr Grass at Moffitt and Dr SANTOSH YAJNIK at Medical College of WI who just came over from Univ of Chicago.

22. Gleason 9. ADT for 7 years, failed zytiga after being on it for 3 years, failed 3 rounds of doxetaxol this past fall. Radiation treatments to pelvis in 2021 and spine, hip in 2023. Had BRCA mutations so tried Lynparza January 24.

how soon after starting orgovyx can one test PSA levels.
Doc has written to do a

test after 4-6 weeks but I wanted to do it sooner just to check that this generic version of relugolix is being effective or not. When is the soonest I can get

Magnetic Resonance Spectroscopy
Have contra-indications from multi-parametric MRI (4/2023) and latest serum
PSA
(4.4 ng/ml, 9% free
PSA
). SO recommends retest and another MRI in 6 months. Verifying that prev MRI did not include spectroscopy. Im thinking MRI w spectroscopy is the next step if it wasn't part of previous MRI.

Perineum Irritation / Pain
I was tested on day 8 and my

had already fallen from 8.7 to 2.78. I had been feeling very good with virtually no side effects. My problem is I’ve had this irritation of the perineum for a month or more. It started as a “tingling” feeling, progressed to a burn, now more of a pain.

Estrodiol and PC
My husband has been rediagnosed with NMPC he is on ADT which is holding the Testosterone very low but
psa
shot upto 82. New MO is starting Enzalutimide plus continuing ADT. Not looking forward to the side effects, but will perservere until

drops.
