Hi all

After reading the messages for a long time, I finally took the time to update my profile to describe my history of the disease. That's it

I live in France, I was born in April 1963, I am married and I stopped my professional activity 10 months ago

Diagnosed 09/2019, PSA 6.4, Biopsy: 6 Gleason 7 (3+4) right samples, 1 sample

left side Gleason 8 (4+4) . Radical Prostatectomy 11/2019.

Postoperative Conclusion :

Infiltrating prostatic adenocarcinoma with a single bilateral posterolateral focus, predominant to the right of Gleason score 4 + 5 = 9 - Grade Group 5.

- Presence of tumor perinerve sheaths.

- absence of vascular embolus.

- presence of extra-prostatic extension (seminal vesicle and right posterolateral peri-prostatic tissue over 12mm).

- Presence of invasion of the right and left seminal vesicles on the large slices with limits in the tumor area.

- Absence of invasion of the bladder neck.

- Healthy apex

- inked resection limits in the tumor area posterior to the prostate and adjacent to the seminal vesicles.

- Absence of lymph node metastasis (0/10)

TNM Classification: pT3b N0 R1

Postoperative PSA 0.01

PET scan : There was no detectable macroscopic tumor remnant in the radical prostatectomy compartment, but a 5 mm lymph node was invaded just anterior to the right hypogastric pedicle, with no visceral or lymph node involvement. Discrete but very suspicious activity of a 5 mm osteocondensation of the anterior segment of the right ischium, accompanying a condensation (3 mm) without PET translation currently, but to be monitored, on the left iliopubic branch, in favor of oligo-metastatic bone involvement.

Treatment decided: Enantone injection

PET Scan: Metabolic extinction of right internal iliac lymphadenopathy with virtual disappearance of condensation of the right ischium. Overall stability of the left iliopubic condensation, which remains generally non-active and not very suspicious Stability of the rest of the examination with no appearance of new lesions.

Treatment decided: Enantone + Erleada Injection

Has radiotherapy been considered? There might be a chance of cure.

Unfortunately, the spot on the right ischium vanished with hormone therapy indicating it was a metastasis. While it can't be cured, radiation to the pelvic lympmh nodes and SBRT to the spot on the right ischium may slow progression.

My oncologist needs to talk to the radiologist about doing

an intervention. I wonder if this makes sense for the moment.
