Perineum Irritation / Pain

I’m Stage 4A prostate cancer (spread outside prostate to lymph nodes only) in week 3 of treatment. I’m doing Lupron shots every 3months + Abiraterone with Prednisone daily, will start radiation in a couple weeks. I was tested on day 8 and my PSA had already fallen from 8.7 to 2.78. I had been feeling very good with virtually no side effects. My problem is I’ve had this irritation of the perineum for a month or more. It started as a “tingling” feeling, progressed to a burn, now more of a pain. I feel it off and on all day now but it’s very pronounced at night as a precursor to peeing and really disrupts my sleep. My doctors said it wasn’t PC related as there weren’t typically symptoms. And as I said, it started as a minor issue. It felt very similar to a UTI or enlarged prostate issue from my past. I did a blood test for infection that came back negative. Now I’m taking Cipro and Tamsulosin but after 3 days it’s no better. It’s hard not to worry it’s PC related. Any similar experiences?

Do you have a history of prostatitis?

No long history maybe once or twice some years ago…before I had the procedure to open the prostate for better urination.

I don't think it ever goes away completely. It almost always relapses and remits. Perineum pain is a typical symptom.

Typical of PC or typical of a treatment? Is there anything to alleviate? Will the Cipro eventually work? You don’t think it’s a symptom of some further spread?
