ADT/Exercise and QoL thanks
Just an anecdotal story to illustrate how quickly muscles can respond, up or down, when on
ADT
. Exercise has been critical in helping me feel good and not be battered by
ADT
(other than those darn hot flashes!).

Low hemoglobin, and foot neuropathy common with ADT?

ADT before Surgery?
I have a prostatectomy which is probably 6-8 weeks in my future. My outlook is that metastasis in imminent. (T3bM0n0 and no evidence of cancer outside the gland as of May 25 PSMA PET scan, but I am told I am intermediate/unfavorable and my PSA went from 10 to 12 in only a month.) Today is August 18.
I have a prostatectomy which is probably 6-8 weeks in my future. My outlook is that metastasis in imminent. Treatment Decision: Now considering surgery
Brachy's full recommendation is to start
ADT
immediately, then surgery, then EBRT and continuance of
ADT
up to 24 months in total. To be clear he is not saying lets do surgery and see where your PSA goes afterwards but rather surgery, 6 weeks of recovery and then EBRT all the while staying on

.

Urinary bother; is Flomax a solution?
SBRT plus
ADT
for 12 months after clean PSMA and 3+4, 4+3, and 4+4. Still up peeing 3 times a night after 4.5 months post radiation. (Down from 8 times). Does anyone know if Flomax will help? Or are there other solutions?

Husband has passed
He was never in pain since he started

. He became extremely weak, fatigued and only wanted to sleep…23 out of 24 hrs. When he was awake, tho, he had such beautiful humor.

ADT and Headaches
To all, I may have mentioned it before, but I've had low-level but disruptive headaches since I started
ADT
along with low RBC/Hemoglobin/Hematocrit. Also as a diabetic, I've been on metformin for many years, thus depleting my B12 levels.

A 4 Year Journey
Had 20 sessions of radiation to the prostate bed (aka-fossa) without
ADT
in August/September 2022 . My PSA was again undetectable until May 2023, when it rose to 0.19. Had 5 sessions of radiation in June 2023 in addition to 6 months of oral Orgovyx, that ended in November 2023.

My own PSA values over time: 70% drop in just 3 months on Dutasteride and no ADT
That's a 70% decrease in PSA, with no
therapy! Dutasteride decreases
DHT
levels by more then 90%, which is why there is such a dramatic decrease in PSA levels.

dilemma ADT duration
Now been on

(Zoladex) for 10 months (or 8 months adjuvant). There is a suggestion that I stop

ADT + darolutamide + docetaxel (ARASENS trial) has anyone tried it?
After less than one month of regrouping and
ADT
only (decapeptyl - triptotrelin, which got my PSA from 70 to 15), my MO decided to do the ARASENS trial triplet (with my full approval, I was going to propose him this protocol). Has anyone tried it? Any suggestion?
