ADT "vacation" terminology questions

I see a lot of posters here who say they do ADT "vacations." Are there any circumstances when NCCN SoC indicate cessation or interruption of ADT and abi? The term "vacation" implies an inevitable return to ADT. Do you wait for rising PSA or a fixed time to go by?

Temporary ADT can lead to permanent testosterone loss in up to 30% of men. So when we compare sRT-only patients to sRT+temporary ADT, what we're really looking at is a mixed cohort, a part of which are effectively being treated with sRT+permanent ADT.

However, he wants do the SRT w/o ADT. His rationale is that if we do ADT concurrently, there's no way to tell if the SRT was working. Plus, he says that if the SRT fails, then I'll be in the same boat with ADT anyway, so why not wait?

Confused about doublet therapy vs triplet therapy for prostate cancer

However, the clinical trial articles i've read regarding triple therapy, they don't compare ADT + AR verse (doublet) ADT + AR + chemo (triplet), they only compared ADT + Chemo verse ADT + AR + chemo.

Pluvicto after long time androgen deprivation therapy

[i]Continuous long-term ADT significantly reduces the visibility of castration-sensitive PC on PSMA PET/CT. If the objective is visualization of the maximum possible extent of disease, we recommend referring patients for PSMA PET/CT before starting ADT.

ASCO 2023 Poster: Androgen-deprivation therapy and risk of dementia in patients with prostate cancer:Clinical outcomes from real-world data.

Conclusion: This is the largest real-world study describing the association between ADT and the risk of dementia, including over 600 thousand prostate cancer patients. The risk of dementia was significantly higher in patients on ADT, with a 60% increased risk.

Vacation time again

We decided to follow the original plan of intermittent ADT and I’m starting another ADT vacation. This round of ADT had started with minimal SE’s but in the last month or so it’s gotten worse. My energy level has noticeably decreased and the hot flashes have noticeably increased.

Follow up at Duke !!

He confirmed that PSMA PET is now SOC and would have been indicated prior to ADT. Also referenced the STAMPEDE info and affirmed that zytiga in addition to ADT is recommended. We were only started on ADT one year ago.

Does a biopsy of a recurrent LN will help in anything ?

Hi all, short version of my Bio: RP in 2019 followed by 33 sessions SBRT with short term ADT (6 mo). Reccurence in 2021, SRT to one LN in 2022, 5 sessions with short term ADT (6 mo). As soon the ADT stopped the PSA started rising, it´s 0.53 now.

Hey everyone, just discovered this site. Great info and support. I had RP in August of 2022. First psa test was 0.1, month later psa 0.8 month later psa 0.22 Psa remained at 0.22 for next 3 months.My urologist who did RP said it could be "reactive lymph nodes". A CT scan and bone scan revealed nothing

I am wondering if anyone on ADT is taking an ED med and what effects, if any, it has had? Is it less than ideal to throw this on the pile with ADT + ARSi?

ADT Recommendations

I'm probably looking at starting salvage radiation soon with 4-6 months of ADT. The doc suggests starting with Casodex and then two sequential Lupron shots. Is there a better way to do ADT for that period?

Worrying study on long-term ADT (esp Enza)

Is this NED affect of long-term ADT common knowledge? If so, is it particular to Enza? 2. Has anyone ever just stopped ADT and, if so, what was the impact on their PSA? 3. Does anyone have any alternatives to Enza (I was already coming off it because of the horrible, exhausting SEs)?
