I wonder if someone would be good enough to cast an eye over my latest blood test results. I am currently on 125mcg Levothyroxine, and 7.5mcg Liothyronine. I take the T3 all at one in the morning, usually around 5am, and the T4 at bedtime. I also take vit d3/k2, haem iron, Thorne basic b , selenium, and zinc.

With regard to my results, my free T4 seems quite low compared to when I was just taking Levothyroxine, and T3 has dropped also since last results. I have been feeling more tired of late, and have had a cold drag on for nearly a month now. Do you think it’s time for an increase in either Levo or Lio, or both?

All blood test protocols were followed. Thank you very much in advance of all replies.

Hi,

So if you usually take all of your T3 at 5am, how long was it since you last took it when you had the blood test?

Thanks for your reply. Sorry, I forgot to mention that - I’d actually taken the both my levothyroxine and the whole 7.5mcg dose of T3 24hrs before my test. My Liothyronine is prescribed as capsules, so it’s almost impossible to split the dose the day before. So that probably means my T3 is actually higher than it looks?

Yes that's right, there's a very short half life for T3, and very little after 24 hours.

Definitely looks like you could do with an increase though, as to what though is a matter of opinion. Most will suggest trying to increase your T4 first, and this should hopefully bring your FT3 up somewhat. In your case, this seems most sensible if you can't split the T3 you're on, as a jump of another 7.5mcg T3 might not be tolerated well.

Thank you for that. I’ll see about upping the t4 then. I could probably request some 2.5mcg capsules of t3 if needed though, too.

If your last dose levothyroxine was 24 hours before test this result suggests you need next dose increase up by 25mcg per day

Which brand of levothyroxine is your 100mcg and 25mcg tablets

Vitamin D is on low side too

How much vitamin D are you taking

What was ferritin result

At the moment, Teva for 25mcg, and then I take 2 Hillcross 50mcg. I’ve just upped vit d to 4000 a day, rather than 2000. My ferritin is still pretty low. I take 2 three arrows every other day. My stomach tends to protest if I raise my vitamins too much. Thanks for the advice, I’ll look to up my t4 then. My free t4 was nearing the top of the range when I was on 150mcg previously and was advised to decrease. Maybe try alternate days 125 and 150 to begin with and see how that goes?

Different brands

Teva brand upsets many people

Many people find Levothyroxine brands are not interchangeable.

Mercury Pharma make 25mcg, 50mcg and 100mcg tablets 

Mercury Pharma also boxed as Eltroxin. Both often listed by company name on pharmacy database - Advanz

Accord only make 50mcg and 100mcg tablets. Accord is also boxed as Almus via Boots, 

Wockhardt is very well tolerated, but only available in 25mcg tablets. Some people remain on Wockhardt, taking their daily dose as a number of tablets

Crescent levothyroxine. Dosages: 12.5, 25, 50, 75, 100

(Not yet known if all approved dosages are or will be available).

Excipients: lactose monohydrate, maize starch, croscarmellose sodium, gelatin and magnesium stearate.

Lactose free brands - currently Vencamil or Teva

Vencamil is lactose free and mannitol free. originally only available as 100mcg only, but 25mcg, 50mcg and 75mcg tablets became available Sept 2024

Prior to March 2023 Vencamil was called Aristo

Vencamil often very well tolerated/best option for many people

Many patients do NOT get on well with Teva brand of Levothyroxine.

Teva is lactose free, but contains mannitol as a filler instead of lactose, which seems to be possible cause of problems. Mannitol seems to upset many people, it changes gut biome

Relatively new ……Hillcross brand

This is a box, rather than a brand. 50mcg and 100mcg are Accord brand….but beware 25mcg is Teva brand

Once you find a brand that suits you, best to make sure to only get that one at each prescription.

Thanks. I’ll give alternate days a go then. I’ve never noticed a discernible difference between brands, but that doesn’t mean it’s not a factor, so will ask pharmacy to stick to one brand from now on and see if I feel any difference.
