Please could you comment on my blood test results? My dose is 125mcg of levothyroxine. Apparently i need to book a phone consultation to discuss them with my doctor.

I expect your GP wont like your low TSH and if he wants to lower your dose you'll have to argue that your actual thyroid hormones are within range so you are not overmedicated.
FT4 is 70% through range.
FT3 is only 36.36% through range and this is the most important result and tells us if we're overmedicated.
Your conversion is actually not good, FT4 and FT3 should be in balance and in the upper part of their ranges.
As for your hair loss, what is your ferritin level, low ferritin can be one reason for hair loss. Other vitamin deficiencies can cause it as well:
Not necessarily. FT4 70% through range is a pretty good result. Your problem is your FT3 level.
For good conversion to take place we need optimal nutrient levels. Have you had Vit D, B12, Folate and Ferritin tested? I would also try supplementing with selenium l-selenomethionine or a yeast-bound selenium as these are supposed to help conversion (avoid selenite and selenate forms). If nutrient levels are optimal then I'd say you need to add T3 not increase Levo.
Optimal levels are:
Vit D - 100-150nmol/L according to the Vit D Council/Vit D Society.
B12 - top of range for serum Total B12, even around 1000pg/ml. For Active B12 at least 70+, preferably over 100.
You say the only symptom you have now is hair looss. This may be due to low ferritin. Have you had vitamins and minerals tested? Frequently low in Hashi's, particularly D3, B12, ferritin, zinc and selenium. I often find my gradual decline in feeling well is hard to identify until my meds are adjusted and after a few weeks I feel better.
