The results from my latest advanced thyroid test are as follows. This is after reducing my Levothyroxine to 87.5mcg for two months. Blood test was taken 7:50am, 24 hours after levothyroxine, fasting and no supplements for a week.
CRP HS 1.02 (0-3)
Ferritin 116 (30-264)
Folate serum 18.3 (7-35)
Vitamin B12 37.5 (37.5-188)
Vitamin D 94.8 (50-250)
TSH 0.007 (0.27-4.2)
Free T3 4.1 (3.1-6.8)
Free thyroxine 23.1 (12-22)
Thyroglobulin 62.7 (0-115)
Thyroid peroxidase antibodies 89.8 (0-34)
Generally, I’ve been feeling better on this lowered dose of Levothyroxine and my T4 levels have really come down but I think from memory my T3 levels have also reduced and this may account for some brain fog and fatigue. I’m also struggling with motivation and concentration. I take some vitamin D but that looks like I might need to increase supplementation (currently on 4000iu) and also looks like I need additional B12 (currently on 250ug).
Do I need to think about adding T3 back in OR just continue to try and get my T4 levels down a bit more? Think from memory T3 might lower T4 though in any case??

Reduce your levo any further and your FT3 will drop below range. You need to take that T3. You have Hashi's, so poor conversion is pretty much to be expected.
That is a very low dose of B12, but as your B12 level is right at the bottom of the range despite taking it, I think you ought to be tested for Pernicious Anemia. That just doesn't look right.
Thank you Greygoose, I was kind of hoping someone would suggest that I was dreading how tired I would feel if I just lowered the levothyroxine again. I start to flag from midday onwards, so any recommendations on a reasonable T3 plan based on my results and symptoms? Perhaps one 2.5mcg midday dose to start with and see how this goes? I’ve learnt the hard way that slow and steady is the way to go!
Practically impossible to do 2.5mcg exactly with 20mcg Thybon Henning tablets mind but I tried 5mcg Morningside and felt pretty terrible at the time. And they are ridiculously expensive! I may try them again if I feel brave enough since I have a pack but for now will stick with Thybon Henning.
My Mum has suffered with pernicious anemia for year… so that looks like something I should get checked out!
Hi Slow Dragon, I’m not vegan or vegetarian, eat plenty of meat and take a multi B vitamin daily which contains B12 250ug. I guess I show these test results to my GP but wondering if he will say “it’s in range. No action required”….?
With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement
A week later add a separate vitamin B Complex
Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.
Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay
IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results
In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

Thank you for all this! I think I’ll try supplementation first, then get another test in…what six months time to re-evaluate my B12? Yes, I’m on T4 only and have been for many months now, perhaps almost a year…I stopped it as my T4 levels were sky high and I just wanted to focus getting that more in range first if that makes sense…
I’m on Mercury Pharma for Levothyroxine which seems to suit me much better than Teva. I will retry Morningside for the T3 and it will be a good test (after not being on T3 for so long) to see if that brand makes me feel awful….or it was just my symptoms at the time.
