I’m stumped and need some help

Recent blood test today

Tsh - -0.01

T4 14 (12 to 22)

I’m on t3 and t4 100 thyroxine and 30mcg t3

Awaiting free t3 levels to be tested

the bloods were taken at 1pm - medication taken at 7am

Without my consent they took thyroid test due it with out my permission. I specifically told the them not to take thyroid as will give different results

Anyway it came back

I have been completely exhausted and bowels very slow gastro paresis gastro issues .

Medication 100 thyroxine and 30mcg t3

LDL has increased to 3.7 max is 3.5

Question is before starting t3 I was not converting my level was 2.1 (4,1 to 6.8)

So where should t4 be ideally ? Is it lowered due to T3 and starting HRT

My last SHGB was very high 180 about a month ago

My docs are crap and alone.

do I increase the t4 again to 112mcg ?

awaiting t3 levels and other hormones to be tested

Cholesterol is 6.4 but ratio is 2.7

Hba1c is 34 but I have been testing glucose in the morning and it raises sometimes without food so not sure why

Do I ask for cortisol test ?

Very bloated and exhausted

T3 will almost always suppress TSH, but especially if taking 30mcg

100mcg Levo may be too low a dose

Test should ideally be 24 hours after last dose levothyroxine and day before test ALWAYS split T3 as 3 smaller doses spread across the day with last 1/3rd of daily dose 8-12 hours before test

Test was only 6 hours after taking Levo?

Your Ft4 is only 20% through range

Many, many members on levothyroxine plus T3 find they do best when Ft4 and Ft3 are approx 60-70% through range (when tested correctly as listed above .)

See what Ft3 result is

Suggest you look at possibly decreasing T3 by 5mcg ……..and increasing Levo by 12.5mcg

Blood glucose can raise a bit from things other than food as well, waking in the morning, strong emotions, doing weight bearing exercises. (My son is type 1).

Levo often adjusting, usually increasing after starting HRT

Folate, ferritin and B12 look good

You are likely exhausted due to being overmedicated. The higher your T3 dose the lower your free T4 will go as your body will use the T3 and excrete the T4. I am on 75 mcg levo and 5.5 mcg T3 which is a more physiological dose. My TSH is within normal range. I was initially put on 12 mcg T3 which was WAY too much. My free T3 before starting liothyronine was in the 2.1 range like yours. It's now 3.8 to 4.2 with 5.5 mcg T3 daily.
