Review of thyroid test results

Hi everyone! I haven’t been on here for quite a while and wanted to share some results in an attempt to make sense of what’s going on with me.

After taking 25mcg of Levothyroxine for 10 years, in late 2023, my ME doctor suggested increasing my dosage to 50mcg. I increased slowly over 6 weeks as recommended.

Previous test results:

July 2023 (private tests while still on 25mcg)

Triiodothyronine (FT3) 4.1 3.1 - 6.8 pmol/L

Free thyroxine (FT4) 16.0 12.00 - 22.0 pmol/L

Thyroid stimulating hormone (TSH) 2.97 0.27 - 4.2 mIU/L

Thyroglobulin antibodies (TgAB) 31.5. 0.0 - 115.0 kIU/L

Thyroid peroxidase antibodies (TPOab) 9.0 0.0 - 34.0 kIU/L

Thyroxine (T4) 104.0 59.0 - 154.0 nmol/L

January 2024 (NHS tests done approx 2 months after increasing to 50mcg EOD)

Serum TSH 1.03 0.35 - 5.5

Serum FT4 17.2. 10.5 - 21.0

April 2024 (NHS tests after increasing to 50mcg daily)

Serum TSH level < 0.03 mU/L 0.35 - 5.5 Below low reference limit

Serum free T4 level 26.4 pmol/L 10.5 - 21.0 Above high reference limit

Serum ferritin level 56.4 ug/L 10.0 - 291.0

October 2024 (NHS tests after decreasing to 25mcg daily)

Serum TSH level < 0.03 mU/L 0.35 - 5.5 Below low reference limit

Serum free T4 level 16.4 pmol/L 10.5 - 21.0 Above high reference limit

November 2024 (NHS tests approx 2 months after stopping Levothyroxine)

Serum TSH level < 0.43 mU/L 0.35 - 5.5 Below low reference limit

Serum free T4 level 12.7 pmol/L 10.5 - 21.0 Above high reference limit

February 2025 (private tests approx 4 months after stopping Levothyroxine)

Triiodothyronine (FT3) 3.99 3.1 - 6.8 pmol/L

Free thyroxine (FT4) 12.70 12.00 - 22.0 pmol/L

Thyroid stimulating hormone (TSH) 5.25 0.27 - 4.2 mIU/L

Thyroglobulin antibodies (TgAB) 18.3. 0.0 - 115.0 kIU/L

Thyroid peroxidase antibodies (TPOab) 9.0 0.0 - 34.0 kIU/L

All tests, apart from the April 2024 ones, were done early morning, having fasted and not taken meds or supplements for previous 48 hrs.

As I mentioned in my last post, I was feeling pretty awful when I was on 25mcg of Levo, although that may have been due to my other health conditions (Long covid/ME & hypotension) and I felt a small improvement when my dose went up EOD. However, I started to feel much worse when I increased to 50mcg daily, was getting a lot of palpitations and other symptoms, and losing quite a lot of weight.

After the April tests, my GP reduced my dosage to 25mcg but I continued to lose weight, the palpitations continued and I was feeling really jittery and anxious and my sleep was all over the place, so in October I decided to stop taking Levothyroxine altogether. Since then I’ve felt a lot more stable, my sleep has improved, the palpitations and other hyper-type symptoms have died down and I’ve stopped losing weight, but I’m now not sure what to do regarding meds. My GP only looks at the TSH and the menopause specialist who prescribes my HRT suggests staying off the Levo for now and watching and waiting to see how I feel, retesting every 2-3 months.

NB: Prior to April 2024, I had a long history of iron deficiency and was severely deficient when I was diagnosed with hypothyroidism. I’m currently taking 60mg gentle iron EOD plus Vit B complex, 500mg magnesium gluconate, 200mg selenium, 500mg myo-inositol, and 3000iu vitamin D daily. I also do a vitamin B12 injection once a month.

My HRT was increased in October to 100mcg oestrogen patches, 100mg micronised progesterone and I added Testogel (40.5mg over 8 days)

Any thoughts/guidance would be very welcome! Thanks in advance.

Sorry you haven’t had any replies

TSH over 5 and Ft4 at bottom of range suggests you need to be on Levo

Suggest you either stay offf Levo and retest FULL thyroid and vitamin levels in another 2-3 months

Or start back on 25mcg levo and retest in 2-3 months
