hi there,

I have been struggling with what I thought was Peri menopause/menopause symptoms for a while (dizzy spells, headaches, brain fog, dry eyes, fatigue etc) Last year these symptoms got progressively worse with pain under left arm in lymph node and weakness to the point I felt like collapsing and stabbing pains in my head both sides up over my ears and weight gain, cold, constipation, bloating and pains in my both feet. After having a cervical polyp removed I mentioned the symptoms to my gynaecologist who said it wasn’t menopause as my blood indicated I was post and headaches etc tend to improve at this time. I started herbs and vitamins in the hope it would help however I reacted badly and discovered my blood pressure was high (it’s going high to low). I’ve had low blood pressure all my life so this was a surprise to me. My GP referred me to a general physician who told me HRT would solve all my problems and prescribed estrogen patches and progesterone tablets. I took them for a week and had a really bad reaction (shaking all the time, excessive sweating and blood pressure spiking) and called the consultant who told me to stop immediately that something else must be going on with my hormones..Blood tests showed slightly elevated testosterone, total protein and alkaline phosphatase. TSH and T4 in normal range. Vitamin D was at 50 so I started a spray which caused same issue of shaking, blood pressure spiking etc. My consultant now wants to check my thyroid anti body levels and T3 just in case. I’m just wondering if anyone has had a similar experience or symptoms. Any suggestions or advise appreciated.

TSH and T4 in normal range.

Please add actual results and ranges

For full Thyroid evaluation you need TSH, FT4 and FT3 tested

Also both TPO and TG thyroid antibodies tested at least once to see if your hypothyroidism is autoimmune

Very important to test vitamin D, folate, ferritin and B12 at least once year minimum

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high TPO and/or high TG thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Significant minority of Hashimoto’s patients only have high TG antibodies (thyroglobulin)

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Thank you for your reply. The bloods were taken at 10:30am while fasting. See results below:

TSH 2.32 ( 0.27 - 4.20)

T4 Free 17.4 ( 11.9 -21.6)

Ferritin 91 (13 -150)

Folate 14.3 (3.9 - 26.8)

B12 394 (197-771)

Alkaline Phosphatase 114 (35-104)

total proteins 83 (60-80)

Testosterone 1.5 (0.1-1.4)

SHBG 40.4 (27.1-128)

Vitamin 50 (50 or below indicates deficiency)

Cholesterol total 5.2 (0.0 - 5)

TSH is at it's highest before 9am and then drops throughout the day, fT4 doesn't look too bad but really need to see a result for fT3 (the active hormone)

You B12 could do with a boost and I'm thinking the Vitamin 50 is actually Vit D and you certainly need to supplement that along with K2 MK7 to get it above 100

Thank you. Yes vitamin D is 50. I am seeing consultant on Tuesday and will request additional blood tests.

Work on improving low vitamin D, folate and B12

Retest thyroid levels again in 2 months including Ft3 and both TPO and TG thyroid antibodies

only add one vitamin supplement at a time. Then wait 2 weeks to assess before adding another

Starting with vitamin D

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

There’s a version made that also contains vitamin K2 Mk7.

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement

A week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

Low folate

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

Igennus B complex popular option too. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose
