I have a multi nodular goitre and am awaiting ENT assessment.

I had bloods redone as I’m feeling extremely fatigued and have symptoms such as breathlessness, tremors.

my results were:

Total white cell count: 5.2 X10^9/L (normal range: 3.9 - 11.1)

Red blood cell (RBC) count: 3.97

×10^12/L (normal range: 3.88 - 4.99)

Haemoglobin estimation: 125.0 g/L (normal range: 118 - 148)

Haematocrit: 40.1 % (normal range: 36 - 46)

Mean corpuscular volume (MCV): 101.0 fL (normal range: 82 - 98)

Mean corpusc. haemoglobin (MCH): 31.5 pg (normal range: 27.3 - 32.6)

Platelet count: 285 ×10^9/L (normal range: 140 - 400)

Neutrophil count: 3.10 x10^9/L (normal range: 1.7 - 7.5)

Eosinophil count: 0.10 ×10^9/L (normal range: 0.0 - 0.5)

Basophil count: 0.10 x10^9/L (normal range: 0.0 - 0.1)

Monocyte count:

0.60 x10^9/L (normal range: 0.0 - 0.8)

Serum ferritin: 14 ug/L (normal range: 13 - 150)

Serum vitamin B12: 342 pg/ml (normal range: 197 - 771)

Serum folate: 5.3 ng/ml (normal range:

3.0 - 20.0)

bloods taken 6 weeks previous:

Serum free T4 level: 15.3 pmol/L (normal range: 11.0 - 21.2)Serum free T3 level: 4.2 pmol/L (normal range: 3.1 - 6.8)Serum TSH level: 1.75 mlU/L (normal range: 0.27 - 4.20)

Thyroid peroxidase antibod lev: < 4 IU/mL (normal range: - 35)

My MCV is the most notable change, could this be linked to my thyroid?

What time was this blood test please?

For a start your vitamins are horribly low so no wonder you're fatigued. Your FT4 is below 50% and FT3 (the active hormone) 30%.

TSH is not too bad but that may be because your vitamins are so low, this does affect it.

Working on vitamins will likely bring your TSH up and help you get diagnosed.

Ferritin. Make appointment with GP and they should prescribe iron. NICE states a ferritin of 30 is deficient. This alone will be causing you a lot of fatigue and symptoms.

B12 - could really do with a boost. A good quality B complex will help and keep all the B's in balance. This one has all the right ingredients in the right quantity. amazon.co.uk/Liposomal-Soft...

Folate is low. If you get the B complex as above then it contains methylfolate anyway so will bring this level up.

MCV is not linked to thyroid but vitamin levels.

Thank you. It was at 9am. My GP has changed my ENT referral to urgent so hopefully I can get things moving to find out what’s going on from there.

Did they prescribe any iron? You are very low indeed.

I haven’t had a follow up GP appointment yet, I got the results from my NHS app and the secretary called to say they had pushed my referral to urgent and to expect to be contacted by 05/04. My app notes say my ferritin is ‘normal’ so i guess I’ll have to push for the iron.

Yes you will need to push them. GP's are pretty bad at nutrients.

From the NICE guidelines:

In all people, a serum ferritin level of less than 30 micrograms/L confirms the diagnosis of iron deficiency.

Treating that plus you getting B12 and folate better will improve some of your symptoms.

Your MCV is a sign of anaemia due to your folate and ferritin being low, no wonder you're fatigued.
