New blood test after speaking to a doctor about stomach problem.

Any help understand these blood works please. Would be helpful. I’d spoke to GP on Monday and she said she run blood test, and go for a scan as well to see what’s going on. Blood test came back the same day. (But used to this by now) but would be interested to know what is going on. Thanks for any advice.

TSH level (XaELV) 0.86 miu/L [0.35 - 4.94] is the only thyroid result & it’s good. Unfortunately it doesn’t give enough information you need FT4 & FT3 testing too.

ferritin level (XE24r) 38 ng/ml [20 - 204] this is technically in range but far from optimal. Aim to raise to 80.

folate level (42U5.) 2 ng/ml [3.1 - 20.5] very low what has doctors said about this?

vitamin B12 level (XE2pf) 151 ng/L [187 - 883] also below range is Doctor investigating further?

Also need to test vitamin D. (Folate, ferritin, B12 & vitamin D) must be optimal for levo to work well.

Erythrocyte sedimentation rate (XE2m7) 39 mm/hr [3 - 12]. High. Indicates inflammation, infection.

Plasma C-reactive protein level (XE2dy) 13 mg/L [0 - 5] inflammation marker this is high.

Haemoglobin A1c level - IFCC standardised (XaPbt) 38 mmol/mol [20 - 41] (42 is pre diabetes level, currently below this.

globulin level 41 g/L [20 - 39] can be elevated if dehydrated

Platelet count - observation (42P..) 459 10^9/L [150 - 40]? Assume 400 which means a little over range.

The remaining results are in range, but I don’t know if all considered optimal.

Just testing TSH is completely inadequate

How much levothyroxine are you currently taking

Do you always get same brand levothyroxine at each prescription

You MUST test TSH, Ft4 and Ft3 together

ALL thyroid tests should be done as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24hours before test

Only do private test early Monday or Tuesday morning and then post back via tracked postal service

Meanwhile see GP re vitamin deficiencies

vitamin B12 level (XE2pf) 151 ng/L [187 - 883]

B12 is deficient

GP should run full testing for Pernicious Anaemia before starting on B12 injections or daily B12 supplements and folic acid supplements

folate level (42U5.) 2 ng/ml [3.1 - 20.5]

Folate is deficient

GP must prescribe folic acid. Do NOT start folic acid until 48 hours after first B12 injection or supplements

Ferritin is far too low

Request full iron panel test for anaemia

Are you vegetarian or vegan

Are you currently taking any PPI like omeprazole?

When was vitamin D last tested

Are you currently taking any vitamin supplements?

I was dropped from 150 mg to 125 mg. at the beginning of the year, but handle more better on a 150 mg. they keep changing the brands in Tesco. Always 24 hours before blood, I’d take mine at 6am when I wake up. And 8am the bloods was taken medication taken after blood was drawn when going for a blood test. I’ve had folic acid before, but not on a regular base but never B12. Im not vegan or vegetarian either. I’d like chicken, beef and pork. They only give me Omeprazle when I’d get to much stomach pain. But haven’t had a big flareup for this in over a year now. (Was told to take buscopan on Monday. I’d get vitamin D was last taken in January. Endo had a big massive go at the doctors surgery and end up on medication list at the moment but mine is always low since thyroid cancer. ( my dad is the same after his cancer, he had skin and Oesophagus cancer, but he doesn’t have thyroid problem, we odd ball, a few of us had cancer but never the same ones, never made sense.)

Highly likely poor conversion of Ft4 to ft3

This causes high Ft4 and low Ft3

Reducing levothyroxine can lower high Ft4, but leads to even lower Ft3

Low Ft3 leads to low vitamin levels and gut issues usually due to low stomach acid

PPI like omeprazole is to treat high stomach acid

Gut issues are EXTREMELY Common when under medicated

Having dose inappropriately reduced from 150mcg to 125mcg likely lead to multiple vitamin deficiencies as direct result

Never ever agree to dose reduction based on just TSH it’s completely inadequate

When adequately treated many, many people have very low TSH

Essential to ALWAYS Test TSH, Ft4 and Ft3 together

If Ft3 is not over range you’re not over medicated
