Hi everyone, My Dad (77) diagnosed in January w/ stage 4 PC, mets to L5 spine. PET scan showed no other spots elsewhere. PSA was 19 at time of diagnosis, Gleason 10. BRCA negative. PSA is now at 0.2 since starting hormone therapy. Feeling good, no pain. Still works (owns his own company). Started

Concern about having more radiation to prostate after 43 external beam treatments 15 years ago
15 years ago my husband had 43 radiation treatments after PSA reached 50 & his 3rd biopsy showed 1 core to have cancer., Gleason was 3-3. He has had Lupron injections since then. Meeting with our oncologist for castrate resistance metastatic prostate cancer

Can anyone explain why some people have Gleason 8 PC, high PSA and have metastatic disease, yet some with Gleason 9 and 10 have low-ish PSA (as far as PC goes) and no obvious metastatic disease (confirmed by PSMA)?
