Xtandi (enzalutamide) +ADT slows metastases in recurrent men

castration
resistance in recurrent men with rapid PSA doubling times.

Study of AMG 509 in Participants With Metastatic Castration-Resistant Prostate Cancer

Nivolumab and Ipilimumab for Metastatic Prostate Cancer with an Immunogenic Signature: Phase 2 Trial
Linch initiated his presentation by addressing the limited responses observed with immune checkpoint inhibitor monotherapy in patients with metastatic
castration
-resistant prostate cancer (mCRPC) (1). This phenomenon is largely attributed to the prevalence of a "cold" tumor immune microenvironment.

Mistletoe Therapy?
Now that I am officially
castration
-resistant (after only a year on HT), I'm reconsidering other treatment options again. I looked at functional medicine a year ago and decided against it but now I am going to give it a try.

What’s peoples view on this - I know there has been a shift to target
castration
testosterone level post RT of <20 ng/nL rather than the past <50ng/nL as best prognostic level. The question is when you retest and is my concern over the efficacy of the implant reasonable?

Extreme fatigue
What is the point in continuing the Zoladex and Nubeqa as I am already
castration
resistant? Right now this is severely affecting my QOL as I am moving house and have a lot of things to do.

Mortality Probability after first PSA rise vs non-castrate testosterone levels (low vs Normal)
The paper doesn't mention this, but it would be interesting to find out if this Cohort was
castration
-resistant or not. Likely, they are, because the "Low-T" levels are non-castrate levels. https://pubmed.ncbi.nlm.nih.gov/29266181/

E2 Patch
I've read studies of e2 being effective in restoring bone health, as well as maintaing "
Castration
Levels" of Testosterone, with fewer harmful side effects than abiraterone. My MO won't even discuss estradiol (e2 patch), as an option.

Do CRPC cancer cells always or usually act different then CSPC cancer cells, or do they sometimes act similar to CRPC?
If someone is
castration
sensitive (on ADT pause) and their psa is 40 (for example), and they show very little suspected cancer (5mm spot for example) on a PSMA PET scan or an Axumin scan, then is it likely that
castration
resistant cancer show very little (5mm), as well, on a PSMA PET or Axumin scan

Article regarding MCRPC and prognosis….Feedback?
states that “MCRPC is associated with poorer outcomes, with most patients dying within two years of diagnosis (4).Apr 14, 2022” I don’t know if there’s a part of this I don’t want to accept or if I’m being naive, but I don’t want to believe that your life expectancy becomes SIGNIFICANTLY shorter once

Consulting on next steps
My experience is that it is possible to pause both chemical
castration
and apalutamide with the PSA and testosterone being monitored. PSA could increase slowly as testosterone recovers and any sign of disease progression will cause a re evaluation.

nubeqa only for non metastatic?
I was just reading here that nubeqa is approved only for non metastatic
castration
resistant. Husband had horrendous reaction to xtandi so they switched him to nubeqa but he has 2 bone mets? Should this have happened? What are better options?

I dream of a time when we can successfully treat advanced prostate cancer without the horror show of chemical
castration
, and not treat patients in the way paedophiles or rapists are punished in some jurisdictions.

Final STAMPEDE results for Abi with/out Enza when starting ADT.
As with
castration
, Abi & Enza are not cures. It's certainly good to know that Abi + classic ADT might add a couple of years of life to ADT monotherapy, but at age 56 I would not have been impressed.
