Do CRPC cancer cells always or usually act different then CSPC cancer cells, or do they sometimes act similar to CRPC?

If someone is castration sensitive (on ADT pause) and their psa is 40 (for example), and they show very little suspected cancer (5mm spot for example) on a PSMA PET scan or an Axumin scan, then is it likely that castration resistant cancer show very little (5mm), as well, on a PSMA PET or Axumin scan when the psa level is below 40? Or, is the castration resistant cancer more likely to travel to other locations, maybe even before it gets near the psa 40 level? What's more common, from guys experiences on this site? The 40 psa level is just hypothetical, for comparison purposes.
