Pluvicto / Lu-177 - Do it now or later?
I am a metastatic
castration
resistant prostate cancer patient having undergone most SOC (RP, SR, docetaxel, ADT, Xtandi, Zytiga, Oliparib) steps. Now, like others, faced with very limited remaining SOC options: another round of the dreaded chemo like docetaxel or Lu-177 / Pluvicto.

Gut Microbiome, ADT & Doxycycline
Recent evidence have shown that obesity, insulin resistance, or metabolic disturbances can be associated with changes in the gut microbiome, while animal studies also show that
castration
is associated with changes in the gut microbiome.

Pluvicto increases radiographic progression-free survival (rPFS) before chemo
clinicaltrials.gov/ct2/show/NCT04689828 It tested whether giving Pluvicto increases radiographic progression-free survival (rPFS) in men who: (1) have failed one of the second-line hormonals (abiraterone, enzalutamide, darolutamide, or apalutamide) (2) have not had taxane chemo (3) are metastatic and

"ADT" is a red herring, in that there is no meaningful use of
castration
. I know from experience that testosterone cypionate injected into thigh muscle lingers. It fades away slowly.

After Zofigo, what next?
I've read about the Eclipse trial (Pluvicto, 177Lu-PSMA-I&T versus Hormone Therapy in Patients with Metastatic
Castration
-Resistant Prostate Cancer) that is available here in Orlando. It looks like he'd qualify 12 weeks post-Zofigo.

Docetaxel Extends Enzalutamide Effectiveness
It also shows that triplet therapy may be beneficial in patients who have already progressed to
castration
resistance. We would expect a larger effect if used at the first sign of
castration
resistance. https://www.prostatecancer.news/2022/10/docetaxel-extends-enzalutamide.html

What first?: Provenge or Nubeqa?
The patient/author feels confident that doing provenge very early in
castration
resistance, is the better option.

Is it useful to get a PSMA-PET scan with low volume cancer, newly castration resistant and PSA below 1?
I was
castration
sensitive at the time of all those scans. I am now
castration
resistant and psa six weeks ago was .33 (up from .14&.20 in August). I don't mind doing the PSMA-PET scan again if it is of value.

PSA fluctuations on docetaxel?
(Note: this is his first time being on chemo as he’s never been on chemotherapy before and he has had
castration

Is ADT beneficial after castration resistance sets in?
Is that because the ADT would be slowing some
castration
sensitive cancer, even though the
castration
resistant cancer is growing separately?

Help: Metastasized Castration Resistant Prostate Cancer!
— Yesterday my current doctor of the 5, given me in 16+ months, told me that I had
castration
-resistant prostate cancer and asked me in my second meeting with him if I had a living will. He told me that I may live a year or two at most.
