Struggling with ADT lack of libido
A few days ago I discovered low testosterone can remains low many months in the body after
ADT
, and it is unlikely I will have that many months once the cancer becomes resistant to
ADT
(yet my cancer sucks!). For those who are in lifelong
ADT
, how do you deal with that?

My own PSA values over time: 70% drop in just 3 months on Dutasteride and no ADT
That's a 70% decrease in PSA, with no
therapy! Dutasteride decreases
DHT
levels by more then 90%, which is why there is such a dramatic decrease in PSA levels.
DHT
is the fuel that feeds prostate cancer growth, not testosterone.

Do I need 6th Pluvicto infusion? PSA undetectable for 6 months.
Pluvicto could very well have helped, though
ADT
surely is playing a major role. Once the study data results get published out we can get a better idea of the relative contributions.

On vacation!
Didn't think I would ever go on an
ADT
vacation. My new urologist thinks it would be a good idea since I have several SE from my MDS-RS with the worst being fatigue added to my
ADT
fatigue. Have been on Eligard after radiation for 3 yrs. with PSA undetectable.

Blood Test Result
After being on
ADT
for two weeks, my blood test shows my Testosterone to be 109 (low). I guess this is the goal of
ADT
, to lower my Testosterone. However, in the same test result, the following was flagged as either low or high. Can my
ADT
attributed to the issues flagged by the test result?

If immediately stopping after two years taking Orgovyx how long before testosterone begins to rise?
But I also heard that the longer on any
ADT
the longer the recovery. Or maybe testosterone never recovers? What do you guys think question

Possible SBRT on prostate and lymph nodes?
RO says to do EBRT and
ADT
, w/prophylactic radiation to all lymph nodes. Or, proton therapy w/
ADT
. (Already signed up for consult w/proton @UFPTI/Jax) I'm wondering why not advise SBRT? Can SBRT also be used prophylactically on lymph nodes?

Spot Radiation before Pluvicto?
(We’re not wowed by some of the Pluvicto outcomes on HU in the last few months among older guys who were also heavily treated with
ADT
. If there are some 75 year old+ Pluvicto guys out there, or their caregivers, with good outcomes, please weigh in to give us some encouragement.

Inherited aggressive pc
Brother had psa of 10 at age 59 and dre revealed some abnormality, biopsy revealed pc and he had 20 sessions EBRT and no
ADT
, no problems after, that was 12 years ago, he can't remember his gleason score and has lost his biopsy report. I've been diagnosed favourable intermediate risk.

ADT w/ Heart History
Thoughts on
ADT
with heart disease? Or the course length to hopefully avoid MI or stroke. Or a different course of treatment? Thank you.

What a long strange road it's been.
It crosses the blood-brain barrier, which the other
ADT
drugs do not. So far the f/u MRI's show no evidence of recurrence. The next one is Sat and I just had the blood draw for the PSA. So that's where I am. Considering what I've read of other guys' experience with
ADT
, I'm very lucky.

Decipher and Prolaris and Oncotype...oh my.
Also, seems to be a wide consensus/opinion to go with
ADT
or not. Would be great to hear from those that have these tests and how or if it has impacted their treatment.

Brother's numbers since ADT plus Orchiectomey since diagnosed in February
He had one
ADT
injection right away and opted for an Orchiectomey in May along with Abiraterone. He just had his first Chemo treatment (Docetaxel) on October 1st and the numbers I have posted were before first Chemo. He will be going for Chemo for six treatments.

Are PSMA negative mets usually castrate resistant?
Zytiga/Lupron was effective before my current
ADT
holiday. Is it possible that I could have become castrate resistant while on the vacation? I guess I'll know for sure once I get back on the
ADT
.

Here we go.......
Treatment will be SpaceOAR, SBRT with focal boost and urethra sparing, 5 sessions over two weeks and
ADT
for 6 months as I am intermediate unfavorable. Prophylactic Flomax. Main concern is looking, feeling and acting like a little old man from
ADT
, and I am not ready for that.
