Unsure of best option in early prostate cancer diagnosis.
I have waffled between RT and RP mainly because no guarantee that if I have the RP that RT or
ADT
won’t still be in play at some point. I know that my Urologist has performed many RP’s. Was leaning toward RP but RT is looking more positive as a first treatment option..

PSMA PET Scan????
I'm 4-3/4 years on
ADT
, MO has ordered NO Scans for 2 years even though I've complained of returning bone pain , groin pain, pain in ribs and spine, as well as thigh and pelvic pain.

HDR boost Brachytherapy + IMRT
All RO’s I’ve met with want to wait 2-3 months for the
ADT
to do its thing and shrink the primary tumor and lymph nodes before they that radiation start treatment. One RO I met with wants to treat with just IMRT. Another I met with wants to use HDR Brachy boost + IMRT.

I owe you an update
I began
ADT
with Orgovyx in February, 2024. My PSA was 0.9 by late April. I had five high-dose SBRT treatments in April 29 to May 9. My Medical Oncologist reviewed my results in June and gave me a status of remission on June 7. This was a good birthday present for my 85th year of life on June 10.

She also said at their case review boards, oncologists were all over the map when it came to length of time on
ADT
, the risk-benefits and supporting data. My risk-benefit calculus was weighted towards castrate resistance. So, labs 9 July, consult 13 July.

Vacation from ADT
I searched but only came up with vacations at two years or so. I had a meeting with my MO today. He is conservative but suggested I could stop the meds. I was complaining of brain fog. He said to stop the Abi for about three weeks to see if there is any improvement and if not, stop Lupron. My story

Time to PSA Nadir is related to progression-free-survival

monotherapy (No RP or RT), all metastatic patients, median 74 y.o., GS = 8-10, N=419. A cubic spline curve fit matched the data points very well, with a high correlation coefficient, R^2 = 0.944, and a low p = 0.040.

PSA Risen in Last 6 months
It had been almost undetectable on
ADT
for 10 years, but 6 months had risen to 8.0. Now 6 months later it is 89.0! I am shocked by this sudden rise in 6 months. What could have caused this, and what is our next step in order to lower PSA?

Anyone treated with enzalutamide +cabazitaxel?
I've had sequentially
ADT
+abiraterone; docetaxel+carboplatin; Pluvicto. PSA never got below 3. Latest CT bone scan shows progression in a few areas. RO will zap the progressing areas locally. MO suggested cabazitaxel (1st choice) or enzalutamide (discouraged because cancer is aggressive).
I've had sequentially

a total of 21 months reaching a nadir of 0.001 for some months...based on this study it should improve my odds of having better outcomes. I am now 9 months into my
ADT
vacation; dont know how this will develop.

Only ADT first or add second generation ADT too at start ?
Dad is going to start
ADT
shortly. He has bone mets One MO said to start with Zoladex and after two weeks to add xtandi ( starting 40 mg ) The other MO says to start only Zoladex and will think of xtandi or zytiga later at some point , maybe when Zoladex starts being less effective.

severe neck pain
impossible to move my head to the right…..Wondering if anyone has ever heard of this associated with any of these meds…..I realize there are multiple possible reasons for such pain but nothing has changed in my daily routine besides this……seems curious that it happened contemporaneous with beginning

Radiation PSA, Orgovyx, Bone density
Bone density down considerably( thanks
ADT
) but still normal for age 83. Starting Calcium, vitamin D, and increasing resitance training. Prefer not to take bone strengthening agents since bad SE with Zometa and MO not positive they will do that much good. I look forward to your comments.

Hypogonadism, heat flushing, and testosterone replacement
It predates my cancer diagnosis, so it has nothing to do with the bicalutamide I took during the radiation process, and I've never had any of the other
ADT
hormones that often cause heat flushing.

Where to get Orgovyx?
Starting SBRT , likely next week with
ADT
starting at the time. I got my doc to prescribe Orgovyx rather than Lupron. On Medicare with Part D. Turns out Walgreens and Walmart cannot get Orgovyx. CVS said they could route it through a specialty pharmacy but could take 4 weeks.

IS THIS A PATTERN???? Thoughts and helpful ideas to guide a path
Had 6 x chemo ,5 X radio to prostate,
ADT
through out.PSA was down to 0.08 at one point.1 MARCH23 0.29 25 MAY23 0.08 NOV23 0.12 05 DEC23 0.25 12 FEB24 0.44 started feeling bouts of unreasonable tiredness, think maybe active cancer??? I'm active, do gym, not over do gym.
