Thought I'd update...many of you are familiar with my clinical history (attached). In April, I met with my radiologist and oncologist, we agreed to come off Orgovyx after 12 months plus SBRT to a single PLN node identified in a March 2023 scan.

My oncologist favored six more months, well, pretty sure he wanted 12 more but was trying to get six and then at that point he would have once again said...

Radiation oncologist was supportive of coming off Orgovyx, knowing that I would have labs and consults every three months, have decision criteria on when to image and when to go back on treatment. She also said at their case review boards, oncologists were all over the map when it came to length of time on ADT, the risk-benefits and supporting data. My risk-benefit calculus was weighted towards castrate resistance.

So, labs 9 July, consult 13 July.

I figured the T would recover (I think it will recover some more...), that is an advantage to Orgovyx. I also figured the three months follow up would have an undetectable PSA.

Consult with oncologist went well given the lab results. I know that when my PCa returns (I'm looking for 2-3 years, my instinct says oncologist feels less than that) he's going to push for 24 months Orgovyx and Xtandi along with SBRT if possible, he confirmed. I said I was open to that, let's see what the clinical data says then and what's new in treatment.

Next prostate specific anxiety test, 9 October, until then, enjoy! WIfe and I have a trip to Glacier National Park, Waterton and Banff planned in September.

I’m curious about your first few PSA tests before you had BCR. What was the threshold of those tests? Was it <0.1 or was it an ultrasensitive assay? Thanks for any details on that.

My PSA tests were single decimal through the end of 2018, so yes, <,01

My first USPSA was Feb 2019, all PSA tests after that were USPSA.

Hawk56, I, too, am in this ADT duration debate with my MO and RO. My MO, and the Proton Beam RO that I consulted for a second opinion, both would like to see 24 months ADT post radiation, scheduled to start last week of this month. They cite the Stampede study. I’m proposing 6 months. My RO for the IG/IM RT is more sympathetic. She is worried about castrastion resistance that would force me onto stronger meds sooner than necessary. I’ve already been on Orgovyx going on six months. Recent lab has my PSa .71 from 16.5, and my T is at 23. So the drug is very effective and painless to take. I appreciate the advantage of Org over Lup, so no slight against Org. But the drug has made me feel 10 years older and I don’t recognize myself anymore. I want to get off of it ASAP. Best wishes for your fight against this beast.
