13 months ago I started having chronic penis tip pain with more pain when rubbed. Its really concentrated at the tip on the inside of the urethra and sometimes around the outside. It feels like a constant burning and more burning when touched. There is also occasional pain in the perineum area. The pain us reduced but not gone when I am naked for longer periods of time.
I remember having oral with some jaw clenching around the circumcision line around that time but I cannot remember exactly when. Was prescribed fungal creams then bactrim and cipro then doxy. These did not help. Also had a cystoscopy and blood tests for all stds or other related issues that all came back negative. I also tried physio and that has seemed to helped the most with controlling things but the pain on touch still always hurts the same. I find that sitting makes it worse and sex and masturbation is bearable but to much concentration on the tip causes the same pain. Also wearing underwear hurts. I bought a softer more compressive pair to hold everything in place.
Recently I have received a pudendal nerve block as one doctor guessed it could be nerve entrapment. The doctor said this guess was due to my pain profile and because my penis went completely numb recently when riding a bike for about 30 minutes. The block failed since the genitals never went numb. I will be receiving a penile block soon as a second attempt.
I posted this in another thread but realized that most of the peoples pain there was along the urethra. Mine is really concentrated at the tip. I am wondering if anyone else has had a similar experience and if anyone has any possible solutions?
I am also wondering if this could be a psychophysiological issue since I have had other chronic pains in the passed which have mostly resolved with some physical intervention and lots of psychophysiological work. This is a very difficult condition to live with and I am trying to stay as happy and positive as possible. I appreciate any help

Hello, I to have had a similar experience. It started about 4 years ago for no reason. I had all the test you can think of but all came back normal. I have since been been given duloxetin at 60mg a day. This did take away the pain but after reading about the side effects of duloxetin I decided to ween myself of it. I now take cannabis oil CBD 20% strength and it has worked for me. I have told the doctor and she says to carry on. Do some research on the internet as there are different strengths and it’s a case of trying them to find the one for you. I now have my life back after 4 years and many pills etc.
I live in Lanzarote Spain and I use a local grown CBD oil at 25mil and 20%. It is also from the cannabis plant and not the hemp plant. I tried the hemp variety but it did not work for me.
Hey Spanman, thanks so much for the reply! I will definetely talk about this with my doctor after the penile block. Is there any chance you can describe your pain symptoms? Were they also very concentrated at the tip with some perineum pain and doctors were heading towards the pudendal neuralgia route? I was going through a period of chronic stress and anxiety when it first started, so I do believe that something like cymbalta or CBD might help. Do you take the CBD oil regularly or were you able to get off of it after some time?
Hello, pain was more at the tip but only in the penis. The cymbalta did stop it but only for about 7/8 weeks then they wanted to up the dose as it was not as good. I take the CBD every day, I started with 5 drops under the tongue twice a day. It took about 1 month for it to start working. I now take 8 drops just prior to bedtime. I have to buy it and it costs me 100 euros but lasts me about 5/6 weeks. I now have no problems and no adverse reactions with any other tablets. Hope this helps to convince the doctor to help you with it.
Hey Spanman, so I am just trying to understand your pain location a bit better here. Mine is really specifically at the tip, the rest of the penis does not hurt. I also have some perineal pain but this comes and goes. Is this also similar to your pain? I also recently had an ineffective penile block, meaning the penis went numb but the pain still stayed in the urethra.
I’m experiencing similar pains right at the tip of the penis and any contact with underwear feels like sandpaper. Urologist have diagnosed me with CPPS after antibiotics didn’t do the trick and ive been going to therapy with trigger point massage and it’s helped the perenium pain quite a bit but the penis tip pain is persistent. I did manage to get some magnesium glycenate which I’ve been told should help I just started today so I’ll let you know how it is. Also there’s definitely some issue if the majority of us are feeling this way and for the doctors to say it’s in our head is complete ignorance. Stay strong we’re all in this together
Wow ! I feel kind of the same. The pain i have feels like the tip. Urinating seems to be ok but I have a slight burning sensation a few minutes afterwards. I went to the Urologist who found nothing technically wrong with my urine sample and blood test. He said i have Uritheris but assured me it will go away 95 percent of the time. Told me to wear tighty whities and put nothing on my Schwarz. I have gotten better but not 100 percent. I do feel a light irritation when I walk though. In two weeks i will have a cystocopy
