Could low B12 be causing my symptoms?

Getting symptoms (palpitations, shortness of breath) of B12 deficiency but I am not technically deficient. My B12 is 271 and folate was 4.6 before I began on folic acid tablets to correct this. Should I be taking B12 tablets too?

ECGs and Chest X-Rays came back normal.

Pernicious anaemia, despite the name, is actually an auto-immune disorder of the gut and the most common cause of B12 deficiency.

Serum B12 is a difficult test to interpret and it is quite possible to be deficient at the level you report. B12 absorption in the gut is very specialised and if it goes wrong then it really goes wrong but it tends to take a very long time to develop - years and possibly even decades because your body has significant stores in your liver and it's really when these have been exhausted that the deficiency starts to bite.

Do you know why your folate levels were low? Dietary forms of folate are the best so if it was lack in your diet I would recommend looking to up your dietary intake - fresh fruit and veg - particularly green vegetables. If your diet is rich in fruit and veg that suggest an absorption problem which makes a B12 absorption problem more likely. Its best to avoid supplementing with high doses of B12 before diagnosis if possible as supplementing can make the results very difficult to interpret.

If you eat very little meat/dairy/fish/eggs then B12 deficiency is also a possibility as it is only found in animal products (at least in a bio-accessible form). Supplementing with a supermarket supplement would help in this case so may be worth trying

I have been on PPIs and H2 blockers for a few years for acid reflux so I suspect that may have caused this!

I have started supplementing with a supermarket 10ug b12 tablet. Will that be ok?

acid reflux can be a symptomof both high and low stomach acidity - another condition which affects B12 absorption and can be a consequence of PA

a supermarket supplement won't affect affect diagnosing an absorption problem. If it helps it would suggest dietary rather than absorption problem.

You might need to be taking some extra magnesium. Palpitations can be eliminated by taking about 600 mg per day. Most people don't get enough through their diet. Also, be careful about taking B6 as it can cause nerve damage if you get too much. My Neuro found that I was getting too much, so I had to eliminate my multi vitamin pill. PPI's can cause deficiencies in B12, and you might also need to take CoQ10. Check online about these deficiences caused by PPI's.
