Help with Latest Blood Results Prior to Endo visit

I had my bloods done at the GP back in January and my TSH was 2.27 on 50mcg Levo but I was starting to feel a lot of the initial symptoms of tiredness etc and asked if I could "trial" raising my Levo to 75mcg. My GP was not keen citing I was within the guidelines but I persisted and the Pharmacist agreed. GP is happy to go with whatever the Endo says and my Endo said to keep the TSH around 1 or under which it now is.

I've just had Full Thyroid Panel done with Medichecks as I'm seeing Endo this week. These are the latest results and I'd be grateful if anyone has advice on them particularly the Vitamins, B12, Folate, Vit D, Ferritin etc. They are all much better than 6 months ago.

I have been taking 2 x Three Arrows daily, 1 x Ingennus B Complex daily, 3 sprays of Better You Vit D + K2 daily + 300mg Magnesium (150 Citrate during day and 150 Bisglycinate at night). Test taken at 8.50am, Levo taken 24 hours before and stopped my iron tablets and B Complex 7 days before.

My Ferritin has consistently been low in last 2 years (a year ago it was 27, then 38 in November) so it's slowly going up.

My skin is still incredibly dry and very itchy so wondering if that is vitamin or thyroid deficiency. I am also on HRT so might be those hormones out of whack (minefield 🤯)

I take Accord and have been splitting my tablets to get to 75mcg. Do Accord do 75mcg? I can't have Mannitol but fine with Lactose.

Vitamin D 96 nmol/L (Range: 50 - 250)

Thyroglobulin Antibodies X 421.0 kIU/L (Range: 0 - 115) (**TG in November was 540.40)

Thyroid Peroxidase Antibodies 23.5 kIU/L (Range: 0 - 34)

You’re doing very well on improving vitamin levels

Keep going, especially with ferritin

Free T4 (fT4) 15.6 pmol/L (12 - 22) 36.0%

Free T3 (fT3) 4.7 pmol/L (3.1 - 6.8) 43.2%

You’re ready for next increase to 100mcg daily

Endo may be amenable or may be TSH obsessed

Make sure to have results as % with you

Request “trial” increase

Thank you. I’ve learnt such a lot on this forum particularly from the Ambassadors and have been researching a lot on my own. I’ve been seeing a Kinesiologist for past 12 years and it really helps. I’ve recently learnt that I’m intolerant to Rapeseed Oil which is in so many things and it’s cheap and cutting it out has really helped my inflammation and absorption.

Endo might well be reluctant to agree to raising again so soon after raising to 75mcg but I will cite these percentages and talk to her. She seems amenable if I want to trial increases so we’ll see.

I’ve been feeling very low mood/ depressed recently and just wondered if that would be helped by raising the Levo? If so, will ask Endo to trial raise to help with that and because of the percentage range being under.

you seem to have found a doctor who actually knows how to treat hypothyroid. Your TSH should be about 1 or lower. I always check my families TSH results when they have a blood test because of my medical history because thyroid disease can run in families. My husbands was 0.95 so very good forsomeone who is nearly 70 and doesn’t have thyroid disease . My sons was 0.86 but he is super fit and visits the gym 3 times a week so I am not surprised how good his metabolism is. I am worried my family have inherited this disease from me and I recently did a dna test and found a lot of Irish cousins. One of them has a daughter with celiac disease and my daughter also has this. It’s seems my Irish family have a few people with autoimmune diseases which explains to me where I inherited mine from.

I too have a family history and was surprised that out of 5 children I was the only one not with Hypothyroidism but as my TSH was consistently around 3 or 4 the GP wouldn’t even consider. Sadly I had to go private purely because I felt so unwell I wasn’t even able to work and it was only then that my high TG antibodies were discovered. As soon as so many people say on here, we’re fobbed off by GP’s and doctors until we make a fuss usually because we feel so unwell!

It’s the same story all over the world and I really don’t understand why we have to fight for treatment it’s not like levothyroxine is an expensive drug either. A lot of people are too ill to do anything and I have had periods of being bedridden because of this horrible disease

If the endocrinologist is not keen on increasing your dose, you could also cite the NICE guidelines, which state that a replacement dose should be around 1.6 mcg levothyroxine per kg of weight (stated under initiation and titration). So for example if a person weighs 63 kg, they should be on around 100mcg of levothyroxine. This is of course just a rough guide, as some people might need more and others less, but it provides a bit of leverage, if the physician is reluctant to increase.

You could cite these guidelines and also say that you are still having hypothyroid symptoms, hence you would like another trial increase to see, if that would alleviate your symptoms.
