I've just had my annual TFT. I'm taking 175 levothyroxine 4 days a week and 200 3 days a week. My weight seems to be rising and I feel very lethargic.
The test was taken at 08.45am using your protocol with timing of last meds and fasting.
TFT 0.21 0.35-5.00
T4 14.8 11-23
Vit D 79 50-150
Ferritin 558 12-300 (had an iron infusion prior to an operation )
B12 1082 200-900 3 monthly B12 injections.
Folate 8.7 3.9-19.8. I take vitamin D and calcium as I take steroids so it is to help protect my bones. I don't know what my T3 is although when it was last taken a few years it was 4.8 3.8-6.1. I seem to take a high dose of levo, considering I am only small, weighing around 53kgs. Is it possible I may need a small amount of T3? any info would be greatly appreciated .

Your FT4 is quite low considering the amount of Levo you are taking. Its only at 32% of range.
Do you take your Levo on its own, on an empty stomach, an hour before anything other than water? Its a very fussy hormone and absorption issues can happen if not taken as described.
Do you take your Levo regularly, not forgetting doses?
I know steroids can affect thyroid results , I think they can lower TSH. If your doctor is treating you by TSH then this will never work.
In the first instance you need more Levo.
To know if you need some T3 we would need to see a full panel taken all at the same time - TSH, FT4 & Ft3.
Vitamin D should be around 100 - 150. Buy one that includes vit K2 to help it go to your bones. Some are available in oil or you can take it with an oily meal for better absorption. People having B12 injections do need a B complex to balance out all the B vitamins. Your folate is on the low side.

Thankyou for your answer, I take my levo every night usually around 2am on its own with water. I never miss a dose. I do take a vit B supplement which contains B6 and also folic acid. I just feel that more levo seems an awful large amount for someone of my size and I know my Gp has said not to take more.
It looks like your GP is treating you according to TSH, but as you are taking steroids your TSH will be lowered by that. Best to go by free hormone results.
