Could someone kindly look at the results of my blood tests and give me their thoughts.

Blood tests done at 8.30 a.m. and last T3 and half cap Metavive at 8.30 p.m. the previous day.

FT3 - 6.5 3.10 - 6.80

FT4 - 21.5 11.90 - 21.60

TSH - <0.01 No ref range (but all thyroid reference ranges have all changed - updated 27.09.2023)

serum B12 342 197.00 - 771.00 - No B vits for 2 weeks

Serum folate 9.8 2.90 - 26.80

Serum Vit D 86 25.00 - 120.00 - No vit D for 2 weeks

Serum Ferritin 179 13.00 - 150.00

Serum iron 17.6 6.60 - 26.00

unsat iron binding cap. 44.1 24.20 - 70.10

Serum TIBC 62 41.00 - 77.00

Sat iron binding cap. 28% 15.00 - 50.00%

I am currently taking 75mcg Levothyroxine, 20 mcg Liothyronine and half a cap of Metavive II. Also B complex and Vitamin D.

For a while now I have been sweating a lot on activity (walking) and also palpitations - these can be quite hard ones. I'm wondering if I could be over medicated.

I would appreciate any thoughts on the above. Thank you.

Usually people aim for around the 70% mark for both FT4 & Ft3. Yours are a fair bit above that level.

Free T4 (fT4) 21.5 pmol/L (11.9 - 21.6) 99.0%

Free T3 (fT3) 6.5 pmol/L (3.1 - 6.8) 91.9%

How much Metavive are you taking? I'd suggest dropping it as its just a complication at this point.

Your B12 is very low. What have you been taking for B12? Thats likely to be causing you fatigue and a host of other issues.

Vit D is good.

Folate too low. What are you taking for folate?

Jaydee1507I only take half a cap of Metavive because my FT4 and 3 were both lowish.

I take Ingenous B Complex that contains B12 maybe not enough but I haven't taken it for 2 weeks.

I don't take Folate as I thought it was in the B Complex. I have resumed taking the Complex. Do you suggest adding a Folate supplement?

Your B12 was low after only 2 weeks of not taking it so you may well need the addition of a B12 spray or sublingual.

I'm assuming you are vegan or vegetarian?

Do you take one or 2 tablets of the Igennus B complex? Full dose is 2 tablets per day.

FT4 & FT3 are now on the high side so you need to reduce something.

Okay will reduce Metavive as I have been on the same dose of T3/T4 for over 10 years and has always been low. Dr nor Endo wouldn't increase either. Have been on the Metavive for about a year.

No I am not vegan or vegetarian.

I only take one but from now on I will take 2.

You only need to stop vitamin B complex 5-7 days before BLOOD TESTS

In week before blood test, when you stop vitamin B complex, you want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12 up to day before test

Serum Vit D 86 25.00 - 120.00 -

No vit D for 2 weeks

Don’t stop vitamin D, continue it as per normal, just don’t take on morning of the test

Thank you for the advice. So just take the folate for a week before test and after dropping B vits. But do I start the B12 now and stop a day before test. Is that what you mean?

Yes

With very low B12 you need BOTH vitamin B complex and a separate B12

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement

A week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

This can help keep all B vitamins in balance and will help improve B12 levels too

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

Then in week before test change B complex for a separate methyl folate and continue separate B12
