so posted results a couple of months ago and got my bit D tested also thyroid again and B12 active and folate . These were done through medi check .

B12 active >150 I have PA so SI and 2monthly injections at Drs

Folate 9.0 ul /l >2.9

Free T4 18.1 (11.5-22.7)

T3 4 .2 (3.1-6.8)

TSH 0.20 (0.38-5.33)

Vit D 38.9 (50.1-200) have started supplementing Vit D how long before I should retest ?

Thyroid peroxidase antibodies 137.0 (0-34)

Thyroglobulin antibodies 22.70 (0-115)

The GP did other test and the ones that stood out were :

Serum sexhormone binding 117 (24-111)

Serum testosterone 1.3 (0.0-1.3) so this was top end

Iron

Serum iron 39 (9-30)

Serum transferrin 2.9 (2.0-3.6)

Transferrin saturation index 54% (16-40)

I know iron is a collective but is it ok . I’ve got to make an appointment with an endocrinologist, is there anything I need to get tested or look into before I see him . Thyroid bloods done to protocol . I feel rubbish still and struggle to do things with feeling fatigue.

thanks in advance for any replies

Andrea

Iron way too high. Dangerously high.. Are you supplementing?

How much D are you taking? Did you use this calculator?

was test early morning and last dose Levo 24 hours before test

Which brand of Levo are you taking for 75mcg

Low folate

As you have B12 injections it’s recommended also to supplement a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and may help maintain B12 levels between injections

B vitamins best taken after breakfast

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need separate methyl folate couple times a week

Post discussing different B complex

healthunlocked.com/thyroidu...

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 5-7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

endo.confex.com/endo/2016en...

endocrinenews.endocrine.org...

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12

Low vitamin D

Likely to need to supplement daily and continuously

GP should prescribe 1600iu everyday for 6 months

NHS Guidelines on dose vitamin D required

panmerseyapc.nhs.uk/media/2...

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

leedsformulary.nhs.uk/docs/...

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

mm.wirral.nhs.uk/document_u...

But with Hashimoto’s, improving to around 80nmol or 100nmol by self supplementing may be better

pubmed.ncbi.nlm.nih.gov/218...

vitamindsociety.org/pdf/Vit...

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Test twice yearly when supplementing

Can test via NHS private testing service

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function.

There’s a version made that also contains vitamin K2 Mk7.

One spray = 1000iu

It’s trial and error what dose we need, with thyroid issues we frequently need higher dose than average

No ferritin result

High iron …..are you taking supplements

Never supplement iron without doing full iron panel test for anaemia first and retest 3-4 times a year if self supplementing.

It’s possible to have low ferritin but high iron

Test early morning, only water to drink between waking and test. Avoid high iron rich dinner night before test

If taking any iron supplements stop 5-7 days before testing

How much do you weigh in kilo

75mcg is low dose levothyroxine unless very petite

The GP didn’t prescribe Vitamin D I got one that’s 1000IU take one a day as he didn’t say anything about it .

I will get B complex the one you’ve suggested.

I don’t supplement iron as they keep telling me it’s fine it’s been all over the place .

I was on 100 Levo and it was dropped last year and then again in January so on 75 now . I’m 51.3 kilos after leaky gut and going down to 46 kilos have increased weight ( none gluten now ) . Since the drop in Levi I’ve struggled with fatigue. I was running 20 miles a week . I do anything strenuous as it wiped me out .

Ferritin 28 (15-300) sorry forgot to add that one

Bloods done as suggested before 9 last levo 24 hours before only water .

The GP didn’t prescribe Vitamin D I got one that’s 1000IU take one a day

That’s very unlikely high enough dose

Suggest you get Vitamin D mouth spray and take at least 2 sprays per day

You may need more

Retest after 2-3 months

No other medication or supplements at same as Levothyroxine, leave at least 2 hour gap.

Some like iron, calcium, magnesium, HRT, omeprazole or vitamin D tablets or gels should be four hours away from levothyroxine

(Time gap doesn't apply to Vitamin D mouth spray)

Vitamin levels are low/deficient BECAUSE you are on too low a dose levothyroxine

Low vitamin levels will tend to lower TSH …..especially low ferritin

Ferritin 28 (15-300) sorry forgot to add that one

Ferritin is deficient

Are you vegetarian or vegan

cks.nice.org.uk/topics/anae...

Serum ferritin level is the biochemical test, which most reliably correlates with relative total body iron stores. In all people, a serum ferritin level of less than 30 micrograms/L confirms the diagnosis of iron deficiency

Medichecks iron panel test

Look at increasing iron rich foods in diet

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Updated reference ranges for top of ferritin range depending upon age

healthunlocked.com/thyroidu...

Thank you for your incredible patience while you have been awaiting the outcome of our ferritin reference range review. We conducted this with Inuvi lab, which has now changed the reference ranges to the following:

Females 18 ≤ age < 40. 30 to 180

Females 40 ≤ age < 50. 30 to 207

Females 50 ≤ age < 60. 30 to 264l

Females Age ≥ 60. 30 to 332

Males 18 ≤ age < 40 30 to 442

Males Age ≥ 40 30 to 518

The lower limits of 30 are by the NICE threshold of <30 for iron deficiency. Our review of Medichecks data has determined the upper limits. This retrospective study used a large dataset of blood test results from 25,425 healthy participants aged 18 to 97 over seven years. This is the most extensive study on ferritin reference ranges, and we hope to achieve journal publication so that these ranges can be applied more widely.

Just a thought re folate as I’m in a similar boat with Hashis and SI B12. I’d been taking 5mcg folic acid tablets until my prescription ran out in May then switched to Thorne B Complex which is often recommended here (somewhat more sporadically I confess). After seeing recent folate level of 10 my consultant advised me to take Seeking Health folinic acid lozenges and definitely not the B complex.

andepande profile image
andepande in reply toRealtiger

Thank you so much I’ll look at that . Be nice to have a consultant 🫣 hoping my appointment in January is a decent one at least or it will be private again which I had to do 12 years ago .

Realtiger profile image
Realtiger in reply toandepande

I had to go private in search of a second opinion - best money I ever spent as I think I'd have been signed off work by now. This was advice from the same consultant who asked for a second set of blood work after I had an iron infusion and he taught me to SI for PA back in January. I'm not sure if it's a coincidence but my tinnitus has improved again since switching to the folinic acid lozenges - it had all but disappeared but had started to worsen. Hope you see some improvement and feel better.

andepande profile image
andepande in reply toRealtiger

My tinnitus is horrible at the moment but I’ve been too tired and low to do anything I will get lozenges . Where is best to get them from ( shop or internet) x

Realtiger profile image
Realtiger in reply toandepande

I’m sorry to hear that your tinnitus is so bad - I find it goes hand in hand with fatigue. These are the lozenges I was recommended - start with half a day for a week then daily preferably in the morning as seems they can give you a buzz! supplementhub.co.uk/product...
