Good morning everyone - following my last post when I was suffering a backslide from my previous post when I was feeling so much better - you recommended me to get a full private test - initially to get a T3 reading, but as I was unsure of my Folate /Vit D etc I went for the full Advanced Thyroid test to get a full Thyroid profile.

With these changes I am now thinking that I may be over medicated maybe causing me to still feel unwell - Very gritty dry eyes Still feeling very tired - but no fatigue - achy joints

T4 has increased from 21 - Free Thyroxine X 22.6 pmol/L (Range: 12 - 22)

TSH from 0.01 to TSH X 0.008 mIU/L (Range: 0.27 - 4.2)

T3 from 5.87 to Free T3 6.4 pmol/L (Range: 3.1 - 6.8)

I am not due to see my GP until 5 September - so I would be grateful for your advice on any changes that I should make or consider before then, in the hope of getting back on course -

I am currently taking 125mcg Thyroxine per day in the morning - this was started 16/5/23 and it was three weeks later that I started to feel better - this lasted for three to four weeks before relapsing which was when I had made contact with you last. - I am also quite surprised at the difference in my levels since the previous reading on the 4/7/23 - I think that you are able to access my previous posts ?

I am still continuing to SI B12 EOD and although slow progress I am still finding improvements and am hoping that I will be allowed to continue with this regime - although I do not have a B12 Deficiency diagnosis. It seems that my Thyroid levels/treatment still need tweaking/adjusting so I would be grateful for any help or advice please.

Thank you Slow Dragon - I have a telephone appointment on 17/8/23 with the Clinical Genetics Dpartment - so not sure what that will bring forth.

Your results suggest perhaps a little over medicated

Perhaps try reducing to 112.5mcg daily

Or maybe initially only reduce 4 days week

Retest in 6-8 weeks

You could. That works out as 112.5mcg daily

Use weekly pill dispenser to keep track

Some people are fine alternating doses other people need same dose daily

Many thanks for your help Slow Dragon - I took 100mcg Thyroxine this morning - but going forward I think I will do as you suggest and take 112.5mcg per day and hopefully this might rectify the symptoms and put me back on course again. I wonder if you had any comments please about my Folate and Vit levels please and if it might be sufficient just to take the B Complex with 400ug Folate per day and no longer take the separate Folate capsules ? I am taking one 1000u Vit D 3 Colecalciferol per day.

Your folate is 55% through the reference range i.e. just over mid-range. Since optimal is "upper half of the reference range" you result is already optimal.

You could stop taking the separate folate supplement and just continue with the B Complex. It is possible that your folate levels may drop when you stop taking the extra supplement. I don't know why, but quite a few people have problems maintaining their folate level where they want it, but have no issues maintaining their B12 level.

Your B12 is over the range which is fine for B12 - it isn't toxic. The worst thing you could say about it is that if you are supplementing a separate supplement then you are wasting money by taking it over the top of the range.

Vitamin D 103.0 nmol/L

Your vitamin D is already optimal. Optimal is 100 - 150 nmol/L. Finding the level within that range that makes you feel best is trial and error.
