Hi

I would really appreciate some advice from someone who knows about Thyroid/TSH results. I feel like I am losing my mind thinking about this but I think I may just be stressing myself out unnecessarily and overthinking things since going back into the IVF world.

Long story short, I underwent fertility treatment in 2020/21. At the time my clinic measured my TSH levels and said they were fine. They were around 3 and started rising over treatment to 3.9. Now the majority of the fertility world says below 2.5 for TSH for optimal fertility and closer to 1 is ideal. However my clinic is adament that anything below 4.5 is fine (they are attached to a large teaching hospital in London so do a lot of research). I can't understand why they would say below 4.5 is OK if it wasn't as they report their results and want success. They wouldn't have a policy that was detrimental to their pregnancy rates. But why then does every other clinic seem to say below 2.5? I found it worrying at the time and was about to shell out thousands for a fresh round of IVF so they agreed to put me on 0.25 thyroxine which I started along with my other fertility meds. That round was successful. I kept taking it throughout pregnancy but once I gave birth my GP tested my TSH and said I didn't need it anymore and stopped the prescription.

Now I am about to start on a frozen embryo transfer - it is our one and only embryo so I am super on edge to do everything possible to support the procedure. My TSH was above 3 end of Nov when they tested it at the clinic. I had read on here that thyroid tests should be done in the morning around 9am before eating so I organised to get the full thyroid Function test from medichecks. These are the results I just got in:

TSH 3.44 mU/L (Range: 0.27 - 4.2)

Free T3 4.81 pmol/L (Range: 3.1 - 6.8)

Free Thyroxine 14.0 pmol/L (Range: 12 - 22)

Thyroglobulin Antibodies 10 IU/mL (Range: < 115)

Thyroid Peroxidase Antibodies 9 IU/mL (Range: < 34)

CRP HS 0.42 mg/L (Range: 0 - 5)

Ferritin 60.5 ug/L (Range: 13 - 150)

Folate - Serum 37.36 ug/L (Range: > 3.89)

Vitamin B12 - Active X >150 pmol/L (Range: 37.5 - 150)

Vitamin D 95 nmol/L

Based on these results should I be pushing the fertility clinic to go back on a 25 dose of thryoxine to get my TSH down?

Does anyone know if TSH over 3 is bad for the results of a FET or if it is more likely to impact natural attempts to get pregnant? I have very low AMH so the thought of going through further fresh rounds of IVF is a daunting (but likely) prospect. Any advice or help would be greatly appreciated.

Think you should be pushing for starting on 50mcg levothyroxine

Levothyroxine doesn’t top up failing thyroid it replaces it.

GP will usually think a TSH under 5 is probably “fine”

You need to explain your situation

The first trimester embryo relies on you for thyroid hormones, after this they have their own thyroid

It's so frustrating having medical professionals fob you off and having to beg for something. I am hoping the Fertility Dr will be more flexible than my GP since it is private and I am paying a lot.

Your TSH is higher that average because your thyroid levels are low

Was test done early morning ?

FT4: 14 pmol/l (Range 12 - 22)

Ft4 only 20.00% through range

FT3: 4.8 pmol/l (Range 3.1 - 6.8)

Ft3 better at 45.95% through range

Thyroid is working hard to try to maintain your levels

But both should be at least 50% through range
