Just received results - any advice welcome

Thyroid Stimulating Hormone (TSH) 3.31 mU/L (0.27 - 4.2 mU/L)

Triiodothyronine (FT3) 3.55 pmol/L (3.1 - 6.8 pmol/L)

Free Thyroxine (FT4) 14.3 pmol/L (12 - 22 pmol/L)

Anti-Thyroidperoxidase antibodies 13 IU/mL (0.0 - 34 IU/mL)

Anti-Thyroglobulin antibodies (TGAB) 12 IU/mL (0.0 - 115 IU/mL)

Vitamin D 67 nmol/L (50 - 200 nmol/L)

Active B12 64.5 pmol/L (37.5 - 150 pmol/L)

Ferritin 42.7 ug/L (13 - 150 ug/L)

Folate 5.24 ug/L (>3.89 ug/L)

Just so that we're clear how to interpret your results, you haven't said but I assume you haven't had a diagnosis and not on any thyroid replacement hormone?

If this is the case then comments are much the same as when you've posted previously, your TSH is not high enough for a diagnosis of hypothyroidism, it has to be over 10 for a diagnosis of primary hypothyroidism, or over range (but less than 10) plus raised antibodies for a diagnosis of autoimmune thyroid disease (Hashimoto's), or basically anywhere plus below range FT4 for a diagnosis of central hypothyroidism.

A normal healthy person generally has a TSH of no more than 2, often around 1, with FT4 around mid-range-ish but as we're all different that's not set in stone, and as we're not tested for a baseline in health then none of us know what is normal for us.

Do you have any symptoms of hypothyroidism:

If so then your TSH is telling us that your thyroid is beginning to struggle. If not then maybe those are normal levels for you.

Vitamin D 67 nmol/L (50 - 200 nmol/L)

Recommended is 100-150nmol/L

Active B12 64.5 pmol/L (37.5 - 150 pmol/L)

Below 70 suggests testing for B12 deficiency, we suggest aiming for 100 plus with Active B12

Ferritin 42.7 ug/L (13 - 150 ug/L)

Far too low, ferritin is recommended to be half way through range (so around 82 with that range) although some experts say the optimal level for thyroid function is 90-110ug/L.

Folate 5.24 ug/L (>3.89 ug/L)

As the range is 3.89-19.45 this is a low result. Folate is recommended to be at least half way through range and when no range is given we suggest aiming for double figures.

These aren't much better than when you posted 4 years ago. Have you been supplementing?

Thank you for your reply. I have been feeling worse since posting 4 years ago. Balance issues, dizziness, tinnitus, tingling in hand/feet, numbness, fatigue, joint pain and loads more from the link you have posted. I have been doing a lot of reading up on different things. GP things I'm depressed- easy option. I'm also 51 with regular periods, so GP feels some may be menopausal. I have the following bloods to do privately -

Gastric Parietal Cells

Intrinsic factor abs

Serum Gastrin

MMA

Homocysteine

I have ruled out Thyroid problems with this test. Celiac was negative.

GP won't test Intrinsic Factor as Serum B12 is 276 ng/L. Wouldn't do a full iron panel as I'm a woman- he said it's bound to be low.

I was ferrered by GP to see a haematologist as I have neutropenia , highly likely autoimmune neutropenia. She tested my iron, but the only result in my letter was iron saturation 20.6% which I assume isn't brilliant.

I see you're getting advise from the PA forum about your B12 and as they know far more than me I wont get involved. However a lot of the symptoms you mention above could be low B12.

Joint pain could be low Vit D. Are you supplementing for this? If so what dose, brand and are you taking D3's important cofactors - magnesium and Vit K2-MK7?

When we discuss an iron panel we often refer to optimal levels according to rt3-adrenals.org/Iron_test_... which are:

Serum iron: 55 to 70% of the range, higher end for men

Saturation: optimal is 35 to 45%, higher end for men

Total Iron Binding Capacity (TIBC) or Transferrin: Low in range indicates lack of capacity for additional iron, High in range indicates body's need for supplemental iron

Ferritin: Low level virtually always indicates need for iron supplementation; High level with low serum iron/low saturation indicates inflammation or infection; High level with high serum iron and low TIBC indicates excess iron; Over range with saturation above 45% suggests hemochromatosis

So your saturation is low, it would be useful to have the other results then you can use this calculator to work out the percentage through range:

Result in first box, low limit of range in second box, top limit of range in third box.

If you can't get iron panel done with GP then look at Medichecks Home Iron test which is the full iron panel and can be done by fingerprick or venous blood draw which costs extra.

Thank you. I was unsure about taking Vit D supplements because of Vit K and blood thinning drugs. I'm on Clopidogrel which is an antiplatelet which I'm sure is ok. Will order a full iron panel privately.

About the Vit K, as you are taking a blood thinner then you will need to do some research yourself. Vit K1 is the form that helps with blood clotting and Vit K2 is the form that directs calcium to bones and teeth, but as usual you read up on it and information is conflicting. So please do your research so you can decide what's best for you.
