Help with my results please. Also antibodies have gone down?

Attached are my recent bloods. I increased my levo from 25mg to 50mg 3 months ago and I take 12.5mg of T3 daily. My Thyroglobulin has reduced from 222. I stopped taking Biotin a week before my test which was done before meds or food at 8.30am. I also take vitamin D, zinc, selenium, iron, magnesium, omega, callogen, and vitamin C supplements. I still don't feel great and am losing a lot of my hair (I'm a 42 year old female). Any help interpreting this is appreciated. My GP refuses to increase my levo. Many thanks :)

When did you take your last doses of Levo and T3 before the test? Last dose of Levo should be 24 hours before and last dose of T3 should be 8-12 hours before, splitting your T3 into 2 or 3 doses the day before.

If you took your Levo and T3 that way then your results show that you are undermedicated. Your TSH is too high, especially for someone on T3 where it would normally be low in range or even suppressed. Your FT3 is very low at 21.62% through range and your FT4 is 21% through range.

When on combination hormone replacement where we need FT4 is very individual, some people would be fine with your level, some need it higher, some need both FT4 and FT3 balanced higher up in range.

Does your GP know that you take T3, presumably this is self sourced?

What made you decide to take T3? Looking at your results from last September, they were

TSH: 0.38 (0.27-4.2)

FT4: 15.5 (12-22) = 35%

FT3: 4.05 (3.1-6.8) = 25.68%

These didn't show that you needed to add T3, they showed that you needed an increase in Levo.

Whether you increase T3 or Levo is your choice but you definitely need to increase something.

Hello, thank you for replying again. I took both my T3 and Levo 24 hours before the test. I only take my T3 with my levo in the mornings at around 8am. I don't split my T3 through the day. And I do source my own T3 which I decided to take because both my GP and endo refused to increase my Levo despite my various blood results and I was feeling awful. My GP still refuses to increase my levo and always says my levels are perfect. I'm at a loss.

Looking back at previous posts it was suggested that you could buy your own Levo

and the general consensus of replies to your earlier posts all agreed that it was an increase in Levo that you needed.

Because you took your T3 24 hours before your test you have a false low FT3 result and there's no way of knowing exactly what your FT3 level is. Even if we do take T3 all in one dose normally, it's important that just for the one day before the test we split the dose to get a true picture of our normal circulating amount of T3.

My suggestion would be to obtain Levo one way or another and increase that at this stage to see if you are one of those people who need FT4 at a decent level on combination hormone replacement, but I do have my doubts that you actually need to be on it at all, you may be fine on just Levo.

Thank you. I'm aggrieved to buy my own levo and simply can't afford it as I currently don't work and T3 is quite cheap especially as I only take half a tablet and get 100 in a packet (so lasts me 200 days). For my last results I split my T3 and had the last dose about 10 hours before the test and my results were pretty similar. Clearly something isn't right but it's not fair that both my endo and GP refuse to increase my levo despite what my test results say as well as how I am feeling. If I somehow got more levo, how much would you suggest me increasing by? Thanks

That depends on whether you are going to continue with the T3 or take a step back and go back onto Levo only at an increased dose, which your historic results suggested.

Maximum amount of increase of Levo would be 25mcg at a time in any circumstances and that might be the best idea if you stop the T3. If you continue with the T3 then I'd suggest you start with a 12.5mcg increase in Levo. Whatever you decide retest after 6-8 weeks to check your levels, ensuring that you split the T3 the day before as mentioned above.
