First testing results - would appreciate feedback

After researching hypo relating to my daughter, decided to get checked myself. Had them drawn early this morning. I do not take any medication for thyroid nor do I have any diagnosis at this time, but I sure have a lot of symptoms that could be attributed to thyroid. I am in the US and was able to strong arm a lab order out of my gp.

TPO: 13.3 (0-31)

Tg: <10.0 (0-60)

TSH: 4.53 (.3-3.8)

fT4: 1.02 (.8-1.8)

fT3: 2.86 (2.3-3.8)

rT3: pending - the local lab has to send that test out to their central lab.

vitamin d, 25 hydroxy: 7 (30-100)

B12: 402 (200-900)

Ferritin: 23.6 (20-324)

Folate: 10.5 (>4.5)

It is obvious that I need to get on the vitamin D3 supplementation. I would appreciate any inputs. It seems that these numbers bear some resemblance to my daughter's...

Yes, your Vit D is severely deficient. In the UK our GPs would give us loading doses of D3 to total 300,000iu over a few weeks, then we would retest to see if we had reached the level recommended by the Vit D Council (50ng/ml or 125nmol/L). Once we reached that level we would adjust our dose to a maintenance dose, retesting twice a year to maintain the recommended level.

There are important cofactors needed when taking D3 as recommended by the Vit D Council -

D3 aids absorption of calcium from food and K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems such as hardening of the arteries, kidney stones, etc.

D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds.

Magnesium helps D3 to work. We need Magnesium so that the body utilises D3, it's required to convert Vit D into it's active form. So it's important we ensure we take magnesium when supplementing with D3.

Magnesium comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds

Check out the other cofactors too (some of which can be obtained from food).

Ferritin: 23.6 (20-324)

This is low and it's said the level needed for thyroid hormone to work (our own as well as replacement hormone) is 70, and it's said that half way through range is recommended. With your level so close to the bottom of the range, you might want to get a full blood count and iron panel to see if there is iron deficiency anaemia.

B12: 402 (200-900)

Check the unit of measurement but this is on the lowish side according to an extract from the book, "Could it be B12?" by Sally M. Pacholok:

"We believe that the 'normal' serum B12 threshold needs to be raised from 200 pg/ml to at least 450 pg/ml because deficiencies begin to appear in the cerebrospinal fluid below 550".

"For brain and nervous system health and prevention of disease in older adults, serum B12 levels should be maintained near or above 1000 pg/ml."

I thought in the US that hypothyroidism is diagnosed when TSH reaches about 2.5 - 3.00. It certainly wouldn't get you a diagnosis here in the UK, we have to wait until TSH reaches 10.

Thanks for all the info. This is my first thyroid testing ever to my knowledge and was drawn yesterday. Doc probably hasn’t even seen it yet, so not sure what he will say on the thyroid.

With your Vitd being so low you should ask for a calcium test, if that is high then a parathyroid test.

I see calcium was included in my comprehensive metabolic panel drawn at the same time:

Calcium: 9.1 (8.5-10.3)

Looking back over calcium results in years past, it has been running 8.7-9.1 consistently.
