Right test results this time - would love feedback if possible

After accidentally posting wrong results in my last post (advice which is still super helpful) I have my results from the end of December to share for advice. Many thanks. I am currently on 125mcg and take a range of supplements detailed in last post.

Sorry not got time to read your other post so quick comment, your vit D is too low. The NHS will say its fine but they only supplement to minimum levels.. You need to increase your vit D supplementation. It need to be around the 100 level. Low vit D cause fatigue. aches and non restorative sleep. Expect lovely Seaside Susie will pop in and tell you to increase and by how much. Take care. Also Vit D Council & Grassroots are good websites to look at for info on vit D dosing.

Supplements from last post:

Mid morning I take: Thorne Basic B 1 tablet, 3 sprays of Better You Vit D+K2 , H&B Collagen complex 1 tablet (alternate days), Solgar 200 ug selenium 1 tablet, Bio-ubiquinol Active QH 30mg 1 tablet, Biocare methylfolate 4 drops under tongue, methyl B12 6 drops under tongue. I had been taking a spoonful of H&B liquid iron most days prior to Christmas but haven't restarted this since it ran out. (I probably need to do a new test but funds a bit tight in Jan).

Is there a reason you take separate folate and B12 and not a B complex? The complex helps to keep all the B vits in balance.

D3 should be around 100 so maybe up the D3 spray?

Free T4 (fT4) 21.1 pmol/L (12 - 22) 91.0%

Free T3 (fT3) 4.34 pmol/L (3.1 - 6.8) 33.5%

Your conversion rate is poor. Have you thought about getting some T3 added?

Hi Jaydee, my b12 was low at previous results so following advice I decided to add the extra B12 in their easily absorbed form alongside the Thorne Basic B complex. I have begun to up my vit d spray. My conversion is always poor - I was hoping that going to 125mcg might up my T3. This conversion rate is better than previously. In August my ft4 was 51% through range and ft3 12.97%. This time ft4 is 90% through range and ft3 33.51%. I have considered t3 before but I have battled to get my levo to 125mcg so have followed advice to try and get my vitamin and mineral readings to their best place first.

Also sorry I didn't reply to how I feel. I started 125mcg from 100 in October and feel like I am more generally well. I don't have buckets of energy and do need to pace myself but I think that is just part of having Hashis. I used to get headaches most days but now I get them a couple of times a week when I am more tired. I have some aches each day usually on waking or in the evening but they only feel really flu like when I have been busier. I definitely have to watch my energy levels and plan for it. I have put on about a 11 lbs in the last year with no real dietary changes but I am 51 so know that issues around my age can also be affecting this. My hands and feet are usually on the colder side but not freezing all the time as they have been in the past. Basically I am much better than I was but trying to understand if I am at my optimum

There's still a small amount to do on vitamins. B12, D3 & folate could be a smidge higher. That said I think you're going to be struggling to get FT3 above 50%.

Can you afford a private Endo? Would be faster and easier than NHS.

Having to pace yourself is not part of treated Hashi's, you could do a lot better with a higher FT3. Make a new post to ask for Endo recommendations via PM.

Hello MrsQ20 :

So on the presumption you vitamins and minerals have already been discussed :

We generally feel at our best when our T4 is in the top quadrant of the range as this should in theory convert to a good level of T3 as it is T3 that runs the body and not T4 which is a pro hormone and a storage hormone.

The accepted conversion ratio when on T4 monotherapy is said to be 1 / 3.50 - 4.50 - T3/T4 with most people feeling at their best when they come in this ratio at 4 or under.

You Free T4 at 21,10 and some 91% through its range but with a Free T3 at just 4.34 and some 32% through its range - this shows some disparity and your conversion into T3 compromised.

As for the ratio - if we divide your Free T3 into you Free T4 we get 4.86 - which simply confirms what we have already established and your result moving out from the centre point showing a struggling thyroid and impaired conversion.

The high antibodies show Hashimoto's auto immune thyroid disease and maybe we need to first consider what was said in your previous post.

Thank you very much for taking time to reply. I have read so much around this subject and know that conversion is so key. I have made adjustments such as being gf, limited dairy, no alcohol, huge range of supplement adjustments in past couple of years and ongoing battling with the dr. I just want all this work to try and get me to my optimum health whilst also recognising that rest is also needed. I will look at the link you have provided.
