Erratic PSA and broken bone
I'm resuming chemo this Friday due to PSA spike and staying on
Xtandi
. Any thoughts as to why my PSA was so erratic lately with the sudden drop to 0.023 and then spike to 6.9? Could it be stress related to the broken bone? Thanks.

Pluvicto works but at what cost
Previously on Firmagon,
Xtandi
worked for awhile, nothing with Xofigo and a few rounds of Jevtana didn’t help and caused problems, also had localized XRT - but had to have chemo to qualify for Pluvicto which is a great advanced treatment for many of us After multiple delays getting Pluvicto with PSA

Our Pluvicto story
My husband didn't have success with
Xtandi
or Zytiga. MO feels that cabitaxel is our next best step. Genetic testing has been done with no actionable findings. I know that many are not able to complete all 6 Pluvicto treatments and many aren't able to have any success at all.

Xofigo Update
When it started to fail a year ago they added
Xtandi
. Now that is no longer working and have completed 5 of 6 treatments of Xofigo - which he is tolerating well (blood counts are holding up, with only minor indigestion/diarrhea.

Update on failing Enzalutamide
Brief recent history: a couple of years ago PSA started to rise on the treatment that had worked well (bicalutamide and 3 monthly Prostap injections) So I switched to
Xtandi
just over a year ago. The side effects were horrendous, so I reduced dosage to one-quarter of usual dose.

Note my PSA has risen from 0.27 in November to .49 in April I started

Good news
But for the second time a 3 month period on only 80mg
Xtandi
has failed to have any adverse affect on my progression - I now consider that 80mg is keeping the beast at bay just as well as 120mg was - but the fatigue is much lighter. Makes me happy --- hope it continues for many years.

My Onco said that I needed to stop

Pluvicto Update
I'm also taking
Xtandi
and of course the beloved Lupron. I think it has been better tolerated than Docetaxel but the fatigue may be worse. Not sure if Pluvicto is to blame or the
Xtandi
. I am very weak and tire easily. I did walk a mile the other day but I was wiped out afterwords.

The loss of testosterone has definitely weakened him and the lupron brain fog is very real, (which really sucks because of his adhd)...shortly after starting the
xtandi
he developed a tremor in his right hand.

was discontinued due to severe side effects and he was started on Nubeqa.

2 Questions
I started
Xtandi
on April 15th. My PSA on May 14th was still .7. My MO said this was likely my nadir. I’m not sure if radiation will be approved with a .7 PSA. I’ve received the results of my 2nd PSMA PET scan yesterday.

PC invasive to bladder
My dad started taking
Xtandi
for about a month and his doctor did not suggest any treatment to reduce the bleeding. Have any of you know or experience this situation? Much appreciate if you can share your view and/or experience. Thank you in advance.
