It's Back - The Way Ahead
Triplet therapy - SBRT, Orgovyx and
Xtandi
, the latter being an "ARI," androgen receptor inhibitor, preceding the PCa cells from binding with testosterone, their fuel. He's checking with TRICARE For Life on cost...don't see an issue., I checked with Express Scripts, monthly co-pay $14.

BAT question. A little worried about results.
I was on
Xtandi
as part of a clinical trial at NIH and after 2 years my PSA started climbing and PSMA scan showed metastasis in abdominal lymph nodes and supraclavicular node. Biopsy of the abdominal nodes wasn’t possible but biopsy of the supraclavicular node was inconclusive.

Jevtana prescribed until Pluvicto available to him
He already had Zytiga,
Xtandi
, radiation, cryotherapy, Immunotherapy and Taxotere... running out of options but fortunately, he is still feeling pretty good considering all.

Enzalutamide/Xtandi monotherapy as ADT with fewer side effects than Lupron
Enzalutamide monotherapy can improve the quality of live for advanced prostate cancer patients. Thank you, Prof. Tombal, for promoting this arm in the Embark trial! Enzalutamide does not lower testosterone and estrogen, it even increases the level of these. This avoids the following side effects of

Abiraterone plus Prednisone and 3 monthly injection failing now in 79 year old dad with mets to bone on NHS in UK.
From reading UK NHS guidelines, it seems patients who have taken abiraterone are not then able to switch to
Xtandi
under NHS guidelines. Unless someone else on this site has experienced otherwise. Any advice to give me and him hope would be a blessing.

Xtandi seems very effective
From personal experience and looking at the posts here, it seems
Xtandi
is very effective. The most problematic SE is fatigue.

Stage 4 ~ Now Losing Blood Somewhere
He is going to try
Xtandi
again. That is the jest of his history, but now we learned Monday that he is now losing blood somewhere and they do not know where. He does not want any additional testing to try and locate its origin. His labs are low enough that he is close to requiring a transfusion.

Side effect of a Rash with Xtandi and Apalutamide?
Has anyone gotten a rash from either of these drugs? Has anyone taken an Antihistamine for the rash so they could continue with their medication? Has anyone been successful in lowering their dose? Thank you

Choices after chemo and Xtandi?
Started
xtandi
3 months later psa 330. Cancer has been spreading since April 2022 when psa was 1.42. Had operation to spine due to fracture. On copious amounts of morphine for pain. Met now in 1 lung. Can barely stand let alone walk.
Xtandi
crushed the man he was, along with the cancer.

Can I go on 120mg dosage of Xtandi
I am wondering whether 120mg of
Xtandi
will be as effective in fighting the disease as 160mg, and whether it can keep my psa at undetectable levels. Your suggestions can help. Best to you and every one of you in this fight.

Is Xtandi necessary or even helping me?
My MO started me on
Xtandi
towards the end of October and my T and PSA appear to have pretty much flatlined since then. I've read that "castrate" level T is defined as <20 so if mine is <1, how much additional help is
Xtandi
likely to be giving?

Update to surgery or no surgery
He will be on Lupron every three months and starting
Xtandi
this week. His pain is almost gone just a slight pain in the neck. I hope we will have some scans to see how the tumor is doing. He will have a PSA next Monday.

PSA staying down--would any kind of scan be helpful?
I started on
Xtandi
in June 2022. My PSA was at about .08 before
Xtandi
and after chemo. My question; is there any value in getting a scan to see what is happening with my bone mets, or do I let well enough alone? I am very grateful that my PSA is staying down so far and that I feel good.

So Confused
I start Pluhvicto on the 13th and
Xtandi
the following day. I'm concerned I will feel even worse. Anyone else ever go through this or have any ideas?
