What's next after Xtandi fails?
Seeking other's ideas as I sense his 16 month run with
Xtandi
will be ending soon. His is PSA is fluttering. He hasn't had chemo yet but has lost so much weight that he seems too frail for chemo. Popping a pill is easier but the side effects of
Xtandi
have been brutal for him.

Enzalutamide + Abiraterone Acetate in Bone CRPC
Abiraterone [Zytiga, Abi] & Enzalutamide [
Xtandi
, Enza] both target the androgen receptor [AR] axis, with Abi limiting the supply of androgens & Enza restricting access to the AR itself. It might seem worthwhile to combine the two, but that would depend on how effective the drugs are.

PSA inching up and a couple of "progressions" on Axumin Pet Scan....hmmm.
I'm on Lupron,
Xtandi
and Xgeva right now. They are bad enough. He said we would keep on the same path....he is ordering a Liquid Biopsy from Foundation One because they said my old biopsy didn't have enough to work with.

rising PSA on cabazitaxel !
He has had doxitaxel,
xtandi
, zytiga, Radium 223 ,docetaxel ( second time) and cabazitaxel. His PSA continues to increase and today after his second cabazitaxel it remains at 95. We are so concerned since there has not been any treatment that lowered his PSA.

The more interesting will be next three months as the HighT May resensitize the PC androgen receptors for the heavy smack down that the

is going to bring. After 3 years of ADT, highT felt great.

Long Road, and no legs to walk -
Then came
Xtandi
- (4) pills and after a month he developed horrific headaches, shortness of breath and weak legs. The Dr. had him stop for a week, then lowered the dosage. The symptoms stopped. After a month the symptoms came back and we're much worse.

Firmagon, Xtandi and Radiation
Zoladex 10,8mg 12 weekly for 5 years.Had a PETSCAN ,indicated no metestases outside the Prostate.Oncologist determined that due to rising PSA ,doubled in 3 Months. and became resistant to the Zoladex treatment.The Dr put me on Firmagon,initial triple shot, to be followed by monthly shot as well as

Really wanting a break from the ADT drugs

, and it has worked amazingly well. Been at PSA 0.00 for about 6 months now. After 2.5 years on ADT, Im really ready for a break from the drugs.

Could anyone tell me if it's normal to have bone pains like my husband: back and ribs taking

and if anyone has experience of lowering the dose to 120 mg or 80mg? Could it help him? Would appreciate any reply feeling so worried.

Appeal with the FAA
Was seen at Duke yesterday...Just for the record..my OC second letter pointed out
Xtandi
side effects were based on CT for MPC..and just approved for us guys in July have not yet reached that stage..Also the 1% seizure probability or less came from those with brain cancer, also side effects of this

Failed Xtandi monotherapy, failed Lupron from very first injection, considering Zytiga or clone
Also was curious if any available information of current and upcoming

successors may work even though

Re-sensitizing Zytiga and Xtandi
It seems that Niclosamide also has the potential to re-sensitize both enzalutamide (
Xtandi
) and abiraterone acetate (Zytiga). A few find that Zytiga and
Xtandi
doesn't work for them which is often due to the presence of the splice variant AR-V7.

My options at his time according to the Oncologist would be

or zapping the one left illium Met with 10 Radiation treatments. Now my Onco says to first try the

Both apalutamide and enzalutamide succeed for metastatic hormone-sensitive prostate cancer (mHSPC) - enzalutamide not for high volume mets
New info presented this morning at ASCO - Enzalutamide (
Xtandi
) worked best in men with mHSPC with a [i]low[/i] volume of mets, and in men who did [i]not[/i] plan to have early docetaxel (which is probably much the same thing). Apalutamide worked well regardless of high or low volume of mets.

I asked my Oncologist at MD Anderson what was the average lifespan for someone like me with stage 4 metastatic Prostate Cancer on
Xtandi
and Lupron. He said the average was 3-5. After a period depression from this news, would like to prove him 💀 DEAD wrong.

Why two tests?
The PSA has been slightly rising since it’s
Xtandi
nadir of 1.0 in September. They are having him stay on it while they do scans to check for progression. He had a full body CT scan with contrast last Monday and this coming Monday an NM bone scan.
