The MAO inhibitors phenelzine and clorgyline revert enzalutamide resistance in castration resistant prostate cancer

EPI-7386 now called Masofaniten - Phase II Starting in US and Canada
Hopefully this will get around some mechanisms of
castration
resistance, since hormonal agents (Enza, Appa, Daro) target the AR LBD (Ligand Binding Domain) and eventually AR mutations bypass the LBD for activation rendering the hormonal agents ineffective. -------- Another Drug to watch that targets

Where do I go for alternative to continuous ADT?
I want to an alternative to continuous ADT, whether its IADT, BAT or something else, in hopes of preventing

My MO suspects that I am non-metastatic, castration resistant but will not approve any treatment until metastases develop.
I am not sure I am

Surgical Castration ?
I have been on Eligard for two months. Prior to starting ADT my Triglycerides were 100 (May 2023). After only two months of Eligard they are thru the roof (495). Since I have metastatic PC they tell me that I will be on ADT for life. I am considering surgical orchiectomy since there are fewer metabolic

PSA rising on Triplet Therapy after <18m
As I understand this would be indicative of
castration
resistance. We’d hoped to avoid reaching this point for longer, but it was a blessing to have a period of relative normalcy during this stage of the treatment.

Since there are no long-term studies, the big question this leaves in my mind is are people taking these supplements unknowingly just started a mild form of ADT prematurely and reducing the time for the cancer to start the adaptation to become

Should the dose of Dutasteride be increased?
Since DHT is the actual molecule that "feeds" prostate cancer growth (not testosterone), it follows that reducing DHT as low as possible is as important as reducing testosterone as low as possible when doing chemical
castration
(ADT).

Castration resistant in just a year? Normal or not? Provenge in my future.
I'm already "mildly symptomatic

resistant." PSA tripled in the last 30 days (but still extremely low), after doubling in the prior 3 months. I was diagnosed with stage 4b (mets in multiple bones and nodes) ductal and started ADT 13 months ago.
