Just tell the doctor all about your symptoms, and how they are affecting your life. They will probably take over with a series of questions.

Remember, you're not being interrogated by a lawyer. The doc is there to work with you, and help solve your problems. Be as open as you can. You won't shock him, no matter what you say. They hear this stuff all day, every day. Just relax and be yourself in there.

I've been through psychotherapy in the past. I don;t think you have any need to worry. Professional psychiatrists / psychologists are almost always empathic and caring people who will do their best to make you feel comfortable with them. They're not there to 'test' you in any way, just to see if there are ways they can help you. Try to relax about it and see the psychiatrist as a helper not a threat.
