If you google form GL24 it should bring it up for you, just download it and print it off

By the way it's only pages 21 to 24 incl. that you need to print the rest you can read online

Of course they don't want to give you money - after all no-one is incapacitated by physical/mental problems, they are all scroungers to be maligned by the low paid - the CAB helped my dad with his ESA, assuming you can contact the CAB and they all havn't closed due to lack of funding...

You can also contact welfare rights at your local authority offices. They will take care of all the written work as long as you sign a form for them to act on your behalf. I've just had them deal with my appeal. Good luck. K.xx
