Hey everyone...don't know if anyone has had this experience. I'm about 12 months into my PCa battle...been on Firmagon for most of it (including now). Started on Zytiga last year but had to stop due to elevated liver enzymes, so I switched to Casodex about two months ago, but PSA numbers started to climb from 0.1 to almost 0.3 within about 3 months. Started Xtandi/enzalutamide about 10 days ago and this week's blood test showed AST jumped 22 to 85 within 12 days. (Normal AST range is 13-39). I do NOT have a history of liver issues, except for the Zytiga/abiraterone episode. I read that Xtandi sometimes results is higher liver enzyme numbers, but those rises are moderate and transient. Has anyone experienced rising AST when beginning Xtandi? Did it resolve itself with time?

Started Xtandi and AST spiked!
I was on Casodex and Lupron shots when I started my PC journey. This combo brought my PSA down from a high of 1359 to 2.0 in a mater of 3 months. I continued taking Casodex plus Lupron for 2 years at which time the PC began feeding off the Casodex and PSA started to rise. I then went to Zytiga plus Prednisone. Taking the prednisone led to elevated glucose levels, so that was stopped. I have been on Nubequa and my last PSA was .1
I started Firmagon and Nubeqa at the beginning of treatment in July 2023. I have had Docetaxel chemo and radiation. Have been at <.01 for many months now and have had no unusual blood chemistry issues. I switched from Firmagon to Lupron about 3 months ago so that I could have less frequent shots. The Nubeqa is supposed to have fewer side effects than the others and you may wish to try it. We are all a bit different and side effects will differ. For me, I have had to cut back from1200mg to 600mg daily as the head pressure and cloudy brain were too much. I have not tried anything else. Of the two current treatments, ADT and Nubeqa, the latter seems to be a bigger QOL issue.
Best of luck in your journey.
I stopped using Zytiga about a year ago due to cost. Am in Nigeria, we don't have the affordable version. Still looking for assistance on how to get it.
I was on Lupron and Casodex for many years. Doctor stopped my Casodex (April 2023) and replaced it wit Nubeqa.........My Psa is now the lowest it has been since six years ago.
Note: If you don't mind, please update your bio (age, location, docs. treatment center(s) meds and etc) Thank you and keep fighting those tiny bastards....
Good Luck, Good Health and Good Humor.
